【摘要】 目的：分析桉檸蒎肠溶软胶囊治疗分泌性中耳炎（SOM）的效果及对耳积液白细胞介素-8（IL-8）、IL-10、IL-1β水平的影响。方法：选取2018年1月-2020年1月本院收治的84例SOM患者，采用随机数字表法将其分为对照组和观察组，各42例（耳）。对照组给予鼓膜穿刺术后鼓室注射地塞米松治疗，观察组在对照组基础上加用桉柠蒎肠溶软胶囊治疗。比较两组鼓室导抗图类型、不同语言频率纯音听阈水平、疗效及耳积液IL-8、IL-10、IL-1β水平。结果：治疗后，观察组A型鼓室导抗图占比73.81%，高于对照组的45.24%（P<0.05）。观察组语言频率（500、1 000、2 000 Hz）的纯音听阈值均优于对照组（P<0.05）。观察组总有效率88.10%，高于对照组的69.05%（P<0.05）。治疗后，观察组耳积液IL-8、IL-10、IL-1β水平均低于对照组（P<0.05）。结论：桉柠蒎肠溶软胶囊可提升SOM治疗效果，改善中耳功能与听力水平，降低耳积液IL-8、IL-10、IL-1β水平。

[Abstract] Objective： To analyze the effect of Eucalyptol， Limonene and Pinene Enteric Soft Capsules in the treatment of secretory otitis media （SOM） and its influence on the levels of interleukin-8 （IL-8）， IL-10 and IL-1β in ear effusion. Method： A total of 84 patients with SOM admitted to our hospital from January 2018 to January 2020 were selected and divided into control group and observation group with 42 cases （ears） by random number table method. The control group was given Dexamethasone injection to the tympanum after tympanic membrane puncture， and the observation group was given Eucalyptol， Limonene and Pinene Enteric Soft Capsules on the basis of the control group. The type of tympanic impedance diagram， the pure tone hearing threshold level of different language frequencies， the curative effect and the level of IL-8， IL-10， and IL-1β in ear effusion were compared. Result： After treatment， the proportion of type A tympanogram in the observation group was 73.81%， which was higher than 45.24% in the control group （P<0.05）. The pure tone hearing threshold of language frequencies （500， 1 000， 2 000 Hz） in the observation group were better than those of the control group （P<0.05）. The total effective rate of the observation group was 88.10%， which was higher than 69.05% of the control group （P<0.05）. After treatment， the levels of IL-8，

IL-10 and IL-1β of ear effusion in the observation group were lower than those of the control group （P<0.05）. Conclusion： Eucalyptol， Limonene and Pinene Enteric Soft Capsules can enhance the therapeutic effect of SOM， improve middle ear function and hearing， reduce the levels of IL-8， IL-10 and IL-1β in ear effusion.

分泌性中耳炎（SOM）为耳鼻喉科常见的一种非化脓性炎性疾病，发病率为3.80%～18.10%，病因包括咽鼓管阻塞/功能障碍、免疫反应、感染等，主要以耳闷胀感、耳鸣、听力下降、鼓室积液等为症状表现，若未经积极治疗，易出现鼓室粘连，造成听骨链受损，严重影响听力[1-3]。鼓膜穿刺术可直接清除中耳积液，术后注射地塞米松能够促进残余分泌物消散、吸收，改善患者听力，但病情易反复，整体效果欠佳[4-5]。桉柠蒎肠溶软胶囊为黏液纤毛清除系统恢复剂，可改善黏膜纤毛清除功能，促进黏液排出，并可重建清除防御机制[6]。研究发现，免疫反应损伤所致炎症因子水平变化为SOM的发病机制之一，耳积液白细胞介素-8（IL-8）、IL-10、IL-1β水平与病情严重程度呈正相关，监测三者水平可作为判断治疗效果、预后的有效依据[7-8]。……
