【摘  要】目的：了解老年人医养结合养老意愿和困扰，结合业内研究情况，探讨可行性的医养结合服务供给侧对策。方法：自行设计调查问卷对360位60以上老年人，按照门诊、社区、老年医学科随机原则，在每个环节纳入100-130人进行问卷调查，采用Exce12016进行统计分析。结果：问卷有效率94.99%，调查28个方面、涉及5个以上维度;居家养老占93.26%，慢性病及亚健康61.29%，困扰的问题是疾病、无人照料与寂寞无聊、社區服务不了解;希望社区提供的服务是养生保健、康复护理、文化娱乐。结论：居家养老是老年人的首选养老方式，探索基于老年人需求的新型医养结合养老模式，应该进一步建立依托社区的居家养老，以及与社区相互延伸的主动健康动态管理模式，建立三级医疗机构-社区-居家全链式全方位的智慧医养结合服务、打通环节、建立长效机制，全方位提升社区及居家服务人员的专业素养是满足老年人多元化养老需求、促进医养结合事业科学规范化发展的关键。

【Abstract】Objective：Understanding the elderly's willingness and trouble of medical care combined with pension，Combined with industry research，Discussion on the feasible supply side strategy ofCombination of medical treatment and maintenance.Method：Self designed questionnaire，for 360 elderly people over 60，According to the random principle of outpatient， community and geriatric department，100-130 people are included in each link for questionnaire survey，Statistical analysis with Excel2016.Result：The effective rate of questionnaire was 94.99%，Survey 28 aspects， involving more than 5 dimensions;Home based care accounts for 93.26%，Chronic diseases and sub-health 61.29%，The problems are diseases、unattendedloneliness and boredom and No understanding of community servi;It is hoped that the services provided by the community are health care， rehabilitation care and cultural entertainment.Conclusion：Home-based care is the first choice for the elderly，To explore a new model of combination of medical care and Pension Based on the needs of the elderly，It is necessary to further establish home-based care for the aged relying on the community，and the active health dynamic management mode extending with the community，Establish a three-level medical institution - Community - home full chain and all-round intelligent combination of medical treatment and maintenance、Link up、Establish long-term mechanism，To improve the professional quality of community and home service personnel in an all-round way is the key to meet the diversified pension needs of the elderly and to promote the scientific。
