儿童脓毒症患儿血清可溶性髓样细胞触发受体-1、可溶性CD14亚型、前降钙素和超敏C反应蛋白水平的变化

[摘要] 目的 探讨儿童脓毒症患儿血清可溶性髓样细胞触发受体-1(sTREM-1)、可溶性CD14亚型(sCD14-ST)、前降钙素(PCT)和超敏C反应蛋白(hs-CRP)水平的变化。 方法 选取2015年1月~2018年8月儿科住院治疗儿童脓毒症患儿50例作为观察组,根据其病情程度分为严重脓毒症18例与非严重脓毒症32例。另选择同期在我院体检中心检查的正常健康儿童30例为对照组。比较对照组与观察组、严重脓毒症及非严重脓毒症血清sTREM-1、sCD14-ST、PCT和hs-CRP水平。 结果 观察组患儿血清sTREM-1、sCD14-ST、PCT和hs-CRP水平明顯高于对照组,差异有统计学意义(P<0.01);严重脓毒症患儿sTREM-1、sCD14-ST、PCT和hs-CRP水平明显高于非严重脓毒症患儿,差异有统计学意义(P<0.05或P<0.01)。 结论 血清sTREM-1、sCD14-ST、PCT和hs-CRP水平不仅可用于儿童脓毒症早期诊断,而且可评估患儿病情严重程度。

Changes of soluble triggering receptor expressed on myeloid cells-1, soluble CD14 subtype, procalcitonin and hypersensitive C-reactive protein in the serum of children with sepsis

[Abstract] Objective To investigate the changes of soluble triggering receptor expressed on myeloid cells-1 (STREM-1), soluble CD14 subtype (sCD14-ST), procalcitonin (PCT) and hypersensitive C-reactive protein (hs-CRP) in the serum of children with sepsis. Methods A total of 50 children with sepsis who were hospitalized in the department of pediatrics from January 2015 to August 2018 were enrolled as the observation group. According to the severity of the disease, there were 18 cases of severe sepsis and 32 cases of non-severe sepsis. Another 30 healthy children who were examined at the physical examination center of our hospital during the same period were selected as the control group. Serum sTREM-1, sCD14-ST, PCT and hs-CRP levels were compared between control and observation groups, severe sepsis and non-severe sepsis patients. Results Serum levels of sTREM-1, sCD14-ST, PCT and hs-CRP in the observation group were significantly higher than those in the control group(P<0.01). The levels of sTREM-1, sCD14-ST, PCT and hs-CRP in children with severe sepsis were significantly higher than those of non-severe sepsis patients, and the difference was statistically significant(P<0.05 or P<0.01). Conclusion Serum sTREM-1, sCD14-ST, PCT and hs-CRP levels can be used not only for early diagnosis of sepsis in children, but also for assessing the severity of the disease.

脓毒症是儿科常见的危重病之一,其病情危重,进展快,极易进展为脓毒症休克和多脏器功能衰竭,是导致儿童死亡的主要原因之一[1,2]。如能早期发现及治疗,有助于提高儿童脓毒症的临床治愈率,降低病死率,但目前临床上缺乏灵敏度和特异度均较高的血清学辅助指标[3,4]。可溶性髓样细胞触发受体-1(soluble triggering receptor expresses on myeloid cells-1,sTREM-1)、可溶性CD14亚型(Soluble CD14 subtype,sCD14-ST)、前降钙素(Procalcitonin,PCT)和超敏C反應蛋白(Hypersensitive C-reactive protein,hs-CRP)是近年来新发现或临床上常用的血清炎症相关性指标,在成人脓毒症中应用较多,但在儿童脓毒症应用报道较少[5-7]。本研究观察脓毒症患儿血清sTREM-1、sCD14-ST、PCT和hs-CRP水平,并与正常健康儿童相对照,探讨血清sTREM-1、sCD14-ST、PCT和hs-CRP在儿童脓毒症早期诊断及病情程度评估中的价值,现报道如下。

1 资料与方法

1.1 一般资料

选取2015年1月~2018年8月我院儿科住院治疗儿童脓毒症患儿50例作为观察组。纳入标准[8]:①符合中华医学会儿科学分会急救学组制定的儿童脓毒症的诊断标准[9];②预计生存时间不少于7 d。排除标准[10]:①原有糖尿病、耐糖量或糖代谢异常患儿;②原发或继发性免疫功能低下或长期使用免疫抑制剂者。50例儿童脓毒症患者中原发疾病肺部感染28例,肠道感染12例,尿路感染4例,其他6例。根据其病情程度分为严重脓毒症18例与非严重脓毒症32例。另选择同期我院体检中心检查的正常健康儿童30例为对照组。两组儿童的性别、年龄与体重等均衡可比(P>0.05)。见表1。

1.2 检测方法

所有患儿均在入院30 min内抽取肘静脉血约3~5 mL送检,2500 r/min离心机低温离心15 min,分离出血清并进行分装,冻存于-70℃冰箱备用。采用酶联免疫吸附法测定血清sTREM-1、sCD14-ST、PCT和hs-CRP水平,试剂盒分别购自上海西塘生物公司、苏州市跃亚生物技术有限公司、基蛋生物科技股份有限公司和上海抚生实业有限公司。先在每个检测孔中加入100 μL的样本稀释液,后加50 μL标准品,室温震荡孵育2 h洗涤后加入200 μL耦联抗体,再室温震荡孵育2 h,洗涤后加入200 μL底物,避光孵育30 min,加入50 μL终止液,酶标仪测定各孔吸光值(OD),根据标准品浓度和OD值建立标准曲线,根据标准曲线测定血清sTREM-1、sCD14-ST、PCT和hs-CRP水平。

1.3 统计学方法

采用SPSS19.0 for windows软件进行数据统计分析,符合正态分布的计量资料用均数±标准差(x±s)表示,采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。

2 结果

2.1 两组血清sTREM-1、sCD14-ST、PCT和hs-CRP水平比较

观察组患儿血清sTREM-1、sCD14-ST、PCT和hs-CRP水平明显高于对照组,差异均有统计学意义(P<0.01)。见表2。

2.2 严重脓毒症及非严重脓毒症患儿血清sTREM-1、sCD14-ST、PCT和hs-CRP水平比较

严重脓毒症患儿sTREM-1、sCD14-ST、PCT和hs-CRP水平明显高于非严重脓毒症患儿,差异有统计学意义(P<0.05或P<0.01)。见表3。

3 讨论

脓毒症是指各种致病病原菌或其部分毒素进入血液或组织引起人体严重感染的全身性炎性反应[11,12]。目前临床上对儿童脓毒血症的诊断主要依据是通过血常规、临床表现等对患儿进行诊断,但是目前的诊断方法仅是在疾病发展到一定程度引起患儿出现相关的症状及体征后才能做出正确的判断,不利于疾病的早期诊断及早期治疗。近年来,随着医疗技术水平的发展,儿童脓毒症的诊断及治疗方面也取得了一定的进步,但其发病率和死亡率仍居高不下,是引起儿童死亡的主要原因之一。目前儿童脓毒症早期诊断较困难,临床往往难以区分脓毒症与病毒感染等非感染性疾病,因此,早期诊断、准确评估脓毒症患儿的病情势在必行。血培养目前仍然是脓毒症诊断的金标准,但血培养耗时较长,通常在3~7 d后才获得结果,不仅会耽误患儿的最佳诊疗时间,且存在一定的假阴性率。因此,寻找一种方便快捷,灵敏度和特异度较高的血清标志物在儿童脓毒症早期诊断中尤为重要[13,14]。

sTREM-1是一种2000年新发现的新型炎症激发受体,是一种与炎症相关的免疫球蛋白超家族成员。当机体发生全身炎性反应时,sTREM-1在中性粒细胞和单核巨噬细胞表面的表达率异常明显,通过细胞信号转导通路加快炎症因子在病灶部位集聚和激活,进而放大炎症因子引起的炎症反应产生级联效应,使得炎症反应程度超过机体承受能力,引起炎症性损伤[15-17]。sTREM-1是判断脓毒症患儿严重程度和预后的独立危险因素,是目前早期诊断炎症感染性疾病的新指标[18-20];PCT是用于判断全身感染性疾病标志物的一种血清学指标,其为一种无激素活性的降钙素前体,由甲状腺髓质C细胞分泌,健康生理状态下PCT除肝肾、肺和睾丸组织外其他组织表达较弱或无明显表达,极少释放PCT入血液循环[21-23]。当机体出现严重感染时,PCT可由甲状腺以外许多器官组织大量产生,大量PCT释放进入血液循环,也是目前临床应用较广泛的判断脓毒症早期诊断指标[24-26];CD14是单核细胞/巨噬细胞的一种细胞膜糖蛋白,是天然免疫系统对入侵微生物应激的模式识别受体,有膜结合性CD14和sCD14-ST两种形态。sCD14-ST可激活产生促炎症信号和级联反应,可用于严重感染疾病的早期诊断、其灵敏性和特异性均高于血培养、超敏C反应蛋白、PCT和血白细胞等常规指标[27,28]。

hs-CRP是目前临床最常用的炎性反应标志,是一种由肝脏合成的急性时相反应蛋白,正常情况下人体体液中hs-CRP的含量较低,但在炎症反应,特别是脓毒血症时,hs-CRP水平往往显著增高[29,30]。本研究显示,观察组患儿血清sTREM-1、sCD14-ST、PCT和hs-CRP水平均明显高于对照组,差异有统计学意义(P<0.01),提示血清sTREM-1、sCD14-ST、PCT和hs-CRP水平不仅可用于儿童脓毒症的早期诊断;同时研究还发现严重脓毒症患儿sTREM-1、sCD14-ST、PCT和hs-CRP水平比较明显高于非严重脓毒症患儿,差异有统计学意义(P<0.05),提示血清sTREM-1、sCD14-ST、PCT和hs-CRP水平可评估患者病情的严重程度。

总之,血清sTREM-1、sCD14-ST、PCT和hs-CRP水平不仅可用于儿童脓毒症的早期诊断,而且可评估患者病情的严重程度。由于本研究纳入的病例局限在本院住院患儿,且样本例数较少,期待有大样本和多中心的研究,更能客观、准确地反映血清sTREM-1、sCD14-ST、PCT和hs-CRP水平在儿童脓毒症诊断中的临床意义。
