【摘 要】目的：探讨二种不同的治疗方案对维持血液透析患者继发甲状旁腺功能亢进的发生疗效观察。方法：对2015年1月-2018年12月间将30例维持性血液透析患者是否使用血液灌流治疗分为二组，一组对照组采用高通量透析器治疗，一月两次HDF治疗；二组观察组在此治疗基础上每1-3个月做一次血液灌流（HP）治疗，观察治疗后对血钙、血磷、甲状旁腺激素（PTH）的影响情况。结果：15例采用HP治疗的患者的血磷，血钙，PTH值明显低于对照组。结论：采用HD+HDF+HP治疗模式更能有效升高血钙，降低血磷、PTH水平，是治疗甲状旁腺功能亢进的有效方法。

The observation of effects of two different treatments on secondary hyperparathyroidism following maintenance hemodialysis

【Abstract】The object of this study is to compare two different treatments on secondary hyperparathyroidism in patients in maintenance of hemodialysis by observing the curative effects， side-effects of those two treatments.Method： From 2015. 01.01 to 2017，12， 30， 30 patients in maintenance of hemodialysis were divided into two groups， the treated group （15 patients） and the control group （15 patients）. The 15 patients in control group were treated with blood perfusion therapy. While the 15 patients in treated group were treated with high-flux dialysis device， 1 time per 1-3 months， and blood perfusion therapy. The blood calcium， phosphorus and parathyroid hormone （PTH） after treatment were analyzed and compared between the control group and treated group.Results： the value serum phosphorus， serum calcium and PTH of 15 patients with in treated group were significantly lower than those in the control group.Conclusion： The combined treatment of HD+HDF+HP can effectively promote blood calcium， reducing the level of phosphorus and PTH in patients in maintenance of hemodialysis.

甲狀旁腺功能亢进是由于存在刺激甲状旁腺的因素，特别是血钙、血镁过低，血磷过高，维生素D不足，腺体受到刺激后赠送、肥大，以提高血钙、血镁和降低血磷的慢性代偿性临床表现。长期的甲状旁腺增生，最终导致形成功能自主的腺瘤，引起全身的钙、磷代谢紊乱，高转运骨病进一步发展为骨外转移性钙化、骨骼畸形【1】.美国肾脏病基金会慢性透析患者骨代谢和骨病控制指南（ 简称“指南”） 中要求： 慢性肾脏病 5 期（ CKD5） 患者血清钙应该 控 制 在 2. 10 ～ 2. 37mmol / L，血 清 磷 浓 度 为 1. 13 ～1. 78mmol / L，钙磷乘积为 55mg2 / dl2 （ 4. 52mmol2 / L2 ） ，全段甲状旁腺素 （ intactparathyroid hormone，iPTH ） 在 150 ～ 300pg / ml【2】。我院血透科对2015年1月-2018年12月将30例维持性血液透析患者是否使用灌流器分为二组，观察对2种治疗方案对血钙、血磷、甲状旁腺激素（PTH）的影响，现报告如下：

1 对象与方法
