[摘要] 目的 觀察智能脉冲枪治疗寒湿痹阻型颞下颌关节紊乱的临床效果。 方法 将2016年10月～2017年4月于浙江省慈溪市第六人民医院康复科门诊就诊的56例寒湿痹阻型颞下颌关节紊乱患者应用随机数字表法随机分为观察组（28例）和对照组（28例）。观察组应用SIT6000智能脉冲枪治疗，对照组以听宫、下关、颊车为主穴进行针刺治疗。两组均于入组治疗后7 d应用视觉模拟评分（VAS）、颞下颌关节功能障碍指数（DI）、肌肉压痛指数（PI）及Fricton指数（CMI）评价疗效。 结果 治疗后，两组VAS评分均显著低于治疗前（P < 0.05），观察组DI、PI及CMI评分明显低于治疗前（P < 0.05），且观察组治疗后VAS、DI、PI及CMI评分均明显低于对照组，差异有统计学意义（P < 0.05）。 结论 智能脉冲枪可以明显改善寒湿痹阻型颞下颌关节紊乱患者的颞下颌关节功能，缓解由此导致的疼痛。

Clinical efficacy of impulse IQ in the treatment of temporomandibular joint disorder with syndrome of blockade of cold-dampness

[Abstract] Objective To observe the clinical efficacy of impulse IQ in the treatment of temporomandibular joint disorder with syndrome of blockade of cold-dampness. Methods Fifty-six cases of patients with temporomandibular joint disorder with syndrome of blockade of cold-dampness treated in Outpatient Department of Rehabilitation， Cixi Sixth People′s Hospital from October 2016 to April 2017 were randomly divided into observation group （28 cases） and control group （28 cases） by random number table method. The observation group was treated with SIT6000 impulse IQ， while the control group was treated with acupuncture with the main points of SI19， ST7 and ST6. After treatment for 7 days， the visual analogue scale （VAS）， temporomandibular joint dysfunction index （DI）， palpation index （PI）， and Fricton index （CMI） of the two groups were used to evaluate efficacy. Results After treatment， the VAS scores of the two groups were all significantly lower than those before treatment （P < 0.05）， the scores of DI， PI and CMI in the observation group were all significantly lower than those before treatment （P < 0.05）. After treatment， the scores of VAS， DI， PI and CMI in the observation group were all lower than those in the control group， the differences were statistically significant （P < 0.05）. Conclusion Impulse IQ can significantly improve the function of temporomandibular joint and relieve the pain of temporomandibular joint disorder in patients with temporomandibular joint disorder with syndrome of blockade of cold-dampness.

颞下颌关节紊乱是一种颞下颌关节在运动过程中出现活动受限、疼痛、弹响为主要临床表现的疾病，严重者可影响患者的睡眠，甚则诱发其他情志疾病[1]。西医目前缺乏治疗本病的药物，主要应用糖皮质激素缓解症状，长期用药会导致库欣综合征等副作用[1-2]。中医以祛风散寒、疏筋活络为治法，应用局部针刺治疗本病获得较好的临床疗效，而且可以避免药物所带来的副作用。但局部针刺治疗也存在一些不足，如治疗时存在面部创口且痛感明显，故很多患者依从性差。针对这一情况，本研究应用具有整复关节功能的SIT 6000智能脉冲枪治疗颞下颌关节紊乱，获得了较好的临床疗效，现报道如下：

1 资料与方法
