[摘要]目的 比较小骨窗开颅术和钻孔引流术治疗脑出血疗效。方法 选取2010年1月10日至2016年12月10日期间我院收治的100例脑出血患者，将其随机分为对照组、观察组，每组50例。对照组和观察组分别采用小骨窗开颅术治疗和钻孔引流术治疗。比较两组患者手术相关指标、并发症发生率、预后情况、疼痛评分、格拉斯哥昏迷评分。结果 观察组患者治疗后的手术时间[（32.45±4.89）min]、术中出血量[（52.34±3.69）ml]、血肿清除率（98.00%）、并发症发生率（2.00%）、预后Ⅰ级率（80.00%）、疼痛评分[（1.05±0.25）分]、格拉斯哥昏迷评分[（9.56±0.02）分]均优于对照组（P<0.05）。结论 钻孔引流术治疗脑出血的效果更显著，手术安全性更高。

[Abstract]Objective To compare the effect of small bone window craniotomy and drilling drainage operation in the treatment of cerebral hemorrhage.Methods 100 patients with cerebral hemorrhage admitted into our hospital from January 10，2010 to December 10，2016 were selected and they were randomly divided into the control group （n=50） and the observation group （n=50）.The control group and the observation group was treated with small bone window craniotomy and drilling drainage operation respectively.The operation related indexes，incidence rate of complication，prognosis，pain score and Glasgow coma score were compared between the two groups.Results The operation time [（32.45±4.89） min]，the intraoperative blood loss [（52.34±3.69） ml]，hematoma clearance rate （98.00%），incidence rate of complication （2.00%），grade Ⅰ （80.00%） of the prognosis，the pain score [（1.05±0.25） points]，Glasgow coma score [（9.56±0.02） points] were better than those of the control group （P<0.05）.Conclusion Drilling drainage operation in the treatment of cerebral hemorrhage is more effective，and the operation safety is higher.

脑出血是高血压最常见、严重的并发症，属于心脑血管疾病患者，可危及患者生命安全，而导致脑出血患者死亡的主要原因之一为脑血肿引起的脑部继发性损害[1]。针对此类现象，临床学者常使用手术治疗，清除血肿，缓解脑组织造成的间接损伤，随着微创理念的深入，钻孔引流术、小骨窗开颅术开始广泛应用于临床，由于其各具各的优势，从而导致其疗效出现较大争议[2]。本文比较小骨窗开颅术和钻孔引流术治疗脑出血的临床效果，现报道如下。

1 资料与方法

1.1 一般资料

选取2010年1月10日～2016年12月10日我院收治的100例腦出血患者，将其随机分为观察组、对照组，每组50例。入选标准：①患者经CT检查发现血肿部位，且最大直径>2.5 cm；②患者幕上血肿量>30 ml；③患者均在发病后24 h内完成手术治疗；④患者了解、知情、同意本实验，且签署协议书。排除标准：①无法随访患者；②血……
