噻唑烷二酮类药物对原发性高血压患者颈动脉内膜中层厚度及氧化低密度脂蛋白的作用

[摘要]目的 分析噻唑烷二酮类药物对原发性高血压(PH)患者颈动脉内膜中层厚度(IMT)及氧化低密度脂蛋白(ox-LDL)的作用。方法 选择本院2010年2月~2014年2月符合标准的100例PH患者进行研究。依据治疗措施将患者分为两组:治疗组(n=50)与对照组(n=50)。所有患者均采用常规降压、调脂治疗,治疗组另加上罗格列酮4~8 mg/d,控制血压在140/90 mmHg以下。在0、3、6、12个月测定患者的IMT及ox-LDL水平。结果 治疗3、6、12个月后,患者的IMT明显降低,ox-LDL水平明显下降(P<0.05),且治疗组患者变化比对照组更显著,且差异有统计学意义(P<0.05)。结论 噻唑烷二酮类药物可明显降低PH患者的IMT及ox-LDL水平。

[Abstract]Objective To analyze the role Thiazolidinediones on the intima-media thickness and ox-LDL in patients with primary hypertension (PH).Methods 100 patients with PH meeting the standard in our hospital from February 2010 to February 2014 were selected.Patients were divided into two groups:control group (n=50) and treatment group (n=50).All patients were treated with routine antihypertensive and lipid modulating,and treatment group was plused Rosiglitazone 4-8 mg/d,control of blood pressure was in below 140/90 mmHg.The IMT and ox-LDL level of patients were measured in 0,3,6 and 12 months.Results After 3,6 and 12 months treatment,IMT was decreased greatly and ox-LDL level was decreased greatly (P<0.05).Change of treatment group was obvious compared with control group,and there was a statistical difference (P<0.05).Conclusion Thiazolidinediones can obviously decrease IMT and ox-LDL level of patients.

原發性高血压(primary hypertension,PH)是脑血管病的最重要危险因素,其主要机制是引起动脉粥样硬化(AS)[1-2]。血压升高加速血流紊乱对血管的损伤。内皮功能紊乱导致血浆脂蛋白(氧化修饰型)和炎性细胞(单核和T淋巴细胞)向血管壁浸润,化学趋化因子和生长因子的分泌,导致平滑肌细胞向内皮的移行和增生,并影响氧化低密度脂蛋白(oxidized low density lipoprotein,ox-LDL)水平。ox-LDL主要发挥将胆固醇从肝脏运回全身组织的作用,其在体内过量时,携带的胆固醇便积存在动脉壁上,并最终导致动脉硬化,它在粥样斑块的形成中起重要作用,还可以影响斑块的稳定性[3]。颈动脉内膜中层膜厚度(intirna mediathickness,IMT)是动脉粥样硬化的早期无创性标志物,能很好地预测动脉粥样硬化的发生及病变程度,最近研究发现IMT是预测患者今后发生脑卒中的良好指标[4]。通过超声多谱勒进行准确测量,结果可以评价大、中动脉粥样硬化情况。……
