摘要：目的 观察清热活血合剂治疗血热型寻常型银屑病的有效性和安全性。方法 将112例患者按随机数字表法随机分为治疗组和对照组，其中脱落10例。治疗组（52例）口服清热活血合剂，对照组（50例）口服复方青黛胶囊，疗程8周。治疗前后进行银屑病皮损面积和严重程度指数（PASI）评分、中医主要症状积分、皮肤病生活质量指数（DLQI）积分和血清干扰素（IFN）-γ、白细胞介素（IL）-10测定，并进行血、尿常规和肝、肾功能的安全性检测。结果 治疗组治疗后PASI评分、DLQI积分和中医主要症状积分均较前显著降低（P<0.001），血清IFN-γ水平显著降低（P<0.001），血清IL-10水平显著升高（P<0.001），总有效率100%（52/52）。2组治疗后中医各项症状计分均明显降低（P<0.001），治疗后治疗组皮疹灼热感、瘙痒计分均明显低于对照组，2组比较差异有统计学意义（P<0.001）。安全性方面，治疗组未见检测指标异常。结论 清热活血合剂口服治疗血热型寻常型银屑病疗效明显，可减轻患者的银屑病皮损，提高其生活质量，并改善皮疹灼热感、瘙痒症状，降低血清IFN-γ、升高血清IL-10水平。

Abstract： Objective To investigate the efficacy and safety of Qingre Huoxue Mixture in the treatment of blood heat type psoriasis vulgaris. Methods The patients were divided into treatment group and control group according to random number table method， with 52 cases in treatment group and 50 cases in the control group， 10 cases failed to complete the trial. Treatment group was given Qingre Huoxue Mixture， and the control group received oral administration of compound Qingdai Capsule. The treatment lasted for 8 weeks. PASI score， TCM syndrome integrals， DLQI score， serum IFN-γ and IL-10 were detected， and hematuria routine and liver and kidney function were under safety testing before and after treatment. Results After treatment， PASI score， DLQI score and TCM syndrome integrals in the treatment group decreased significantly （P<0.001）； the expression of serum IFN-γ decreased significantly （P<0.001）； the expression of serum IL-10 increased significantly （P<0.001）， with the total effective rate of 100% （52/52）. After the treatment， TCM syndrome integrals in the both groups decreased significantly （P<0.001）. After the treatment， the rash burning and itching scores in the treatment group were lower than the control group， with statistical significance （P<0.001）. In terms of safety， the treatment group had no abnormal indicators. Conclusion Qingre Huoxue Mixture has obvious curative effect， which can reduce the patients with psoriasis， improve the life quality of patients， and improve the clinical symptoms of rash burning and itching， reduce serum IFN-γ， and increase serum IL-10.

银屑病是一种常见的免疫性皮肤病，临床表现为境界清楚的红色丘疹或斑块，表面覆盖有银白色鳞屑。该病不仅影响患者外观，导致生活质量下降，严重者可出现银屑病关节炎、红皮病等，引起关节畸形甚至危及生命。笔者运用清热活血合剂治疗血热型寻常型银屑病，观察其疗效及安全性，现报道如下。

1 临床资料

1.1 一般资料

共纳入2014年1月-2015年11月本院皮肤科门诊患者112例，采用随机数字表法将患者随机分为治疗组和对照组，其中10例脱落（均为门诊失访）。治疗组52例，其中男性36例，女性16例，平均年龄（40.12±13.93）岁，平均病程5.00（1.25，9.00）年；对照组50例，其中男性29例，女性21例，平均（39.46±14.36）岁，平均病程3.50（2.00，8.25）年。2组患者性别、年龄、病程比较差异均无统计意义（P>0.05），具有可比性。……
