多层螺旋CT灌注成像在脑胶质瘤病理分级中的应用

【摘要】 目的:探讨多层螺旋CT灌注成像在脑胶质瘤病理分级中的应用。方法:选取笔者所在医院神经外科2011年7月-2014年11月收治的52例脑胶质瘤患者为研究对象,根据病理结果分为低级别组(Ⅰ~Ⅱ级)15例、中级别组(Ⅲ级)27例、高级别组(Ⅳ级)10例,分别行CT灌注扫描,比较各组肿瘤实质区及对侧正常脑组织区的血流量(BF)、血容量(BV)、表面渗透性(PS)及相对血流量(rBF)和相对血容量(rBV)。结果:肿瘤实质区BF、BV及PS值均明显高于对侧正常脑组织区(P<0.05);肿瘤实质区BF、BV及PS随着病理分级的增高而增大(P<0.05),而对侧正常脑组织区比较,差异无统计学意义(P>0.05);脑胶质瘤各组最大rBF及rBV随着病理分级的增高而增大(P<0.05),且低级别组最大rBF及rBV均<3,而中、高级别组均>3。结论:多层螺旋CT灌注成像在脑胶质瘤分级中发挥着重要的作用。

【Abstract】 Objective:To investigate the multi-slice spiral CT perfusion imaging in cerebral glioma pathological grading.Method:From July 2011 to November 2014 in our hospital department of Neurosurgery,52 cases with cerebral glioma were selected for the study,based on pathology,the cases were divided into low-level group(Ⅰ-Ⅱ grade) in 15 cases,middle-level group(Ⅲ grade) in 27 cases,the high-level group(Ⅳ grade) in 10 cases,they were underwent CT perfusion scans,blood flow to the tumor in each group and contralateral normal brain parenchyma tissue region(BF),blood volume(BV),permeability surface(PS) and relative blood flow(rBF) and relative volume(rBV) were compared.Result:The tumor parenchyma BF,BV and PS values were significantly higher than the contralateral normal brain tissue area(P<0.05),tumor parenchyma BF,BV and PS with the increase of pathological grade increased(P<0.05),and comparison of the contralateral normal brain tissue areas,the difference was no statistically significant(P>0.05),glioma each group with the largest increase rBV rBF and pathological grade increased(P<0.05),and low-level group maximum rBF and rBV were all <3,while the middle and high-level group were >3.Conclusion:CT perfusion imaging plays an important role in glioma grading.

腦胶质瘤是中枢神经系统最常见的原发性肿瘤,约占所有颅脑肿瘤的45%[1]。按照2007年世界卫生组织神经系统肿瘤分类标准[2],可以分为4级,Ⅰ级为良性,Ⅱ级为低度恶性,Ⅲ级为中度恶性,Ⅳ级为高度恶性。其中,中高级别的胶质瘤占胶质瘤的77.5%,其恶性程度高,血管结构复杂且呈浸润性生长,手术时很难确定肿瘤界限,肿瘤切除不彻底,极易造成患者的复发。胶质瘤患者术前获得较为准确的肿瘤分级信息对手术方案的选择及预后有重要的意义[3]。本文通过对52例不同分级的脑胶质瘤患者术前头颅多层螺旋CT灌注成像参数进行比较,探讨多层螺旋CT灌注成像在脑胶质瘤分级中的应用价值。
1 资料与方法
1.1 一般資料
选取2011年7月-2014年11月河北省沧州中西医结合医院神经外科收治的52例脑胶质瘤患者为研究对象,所有患者均无严重心、肺、肝、肾等功能障碍及碘过敏现象。……
