D-二聚体在诊断下肢深静脉血栓中的应用研究

目的探讨D-二聚体在下肢深静脉血栓诊断中的临床应用价值。方法 方便选取2015年1月—2016年1月该院收治的80例怀疑有下肢深静脉血栓患者分为观察组，将同期80例门诊体检的健康人员分为对照组，采集所有观察对象的血液标本进行D-二聚体测定，并进行超声造影检查明确诊断，对比分析检测结果及D-二聚体检测的诊断意义。结果 观察组患者血浆中的D-二聚体含量（2461.9±1782.6）μg/L明显高于比对照组（168.3±47.3）μg/L，差异有统计学意义（P＜0.05）。观察组中D-二聚体＞500 μg/L有70例，提示有血栓形成，超声明确诊断下肢深静脉血栓65例。结论D-二聚体是下肢深静脉血栓的特异性指标，对早期诊断下肢深静脉血栓有重要的诊断意义。

［Abstract］Objective To discuss the D-dimer the clinical application value of DVT.Methods Convenient selection dividing 80 cases of persons at January 2015— January 2016 who were doubt to DVT into the observation group，Dividing 80 cases of healthy persons into the control group，Collecting all the persons blood samples for D-dimer test，then using Contrast-enhanced to diagnosis the disease.To analysis the test results and diagnostic valiable.Results The D-dimer content of the observation group（2461.9±1782.6）μg/L is obviously higher than the control group（168.3±47.3）μg/L，the results is useful（P＜0.05）.Inthe observation group，70 cases of persons D-dimer content＞500 μg/L，it's prompt has blood clots.The Contrast-enhanced diagnostic DVT 65 cases.Conclusion D-dimer is the specitic indicators of DVT，It's important for DVT to get the correct early diagnosis.

下肢深静脉血栓是临床常见的外周血管疾病，主要见于术后、长期卧床或晚期恶性肿瘤患者［1］。主要因久病体虚、久卧久坐、缺乏运动、刺激性药物、机械性损伤、感染性损伤、纤维蛋白酶溶解能力降低等因素导致血液出现高凝状态、损伤静脉壁、血液循环速度减慢从而形成下肢深静脉血栓。患者主要表现为患肢疼痛、肿胀、压痛、浅静脉曲张、Homans征，遗留皮炎、瘀滞性溃疡等后遗症，降低了患者的生活质量，同时，静脉血栓脱落后会形成肺栓塞，预后较差［2］。所以早期明显明确诊断，并采取有效的溶栓、抗血栓治疗措施可以改善患者的预后。……
