腹腔镜与小切口子宫肌瘤剥除术对体液免疫功能影响的对比研究

目的探讨腹腔镜与小切口子宫肌瘤剥除术对患者体液免疫功能的影响。方法选取2013年1月至2015年1月该科收治的腹腔镜与小切口子宫肌瘤剥除术患者100例，将其随机分为腹腔镜组及小切口组，各50例。采用免疫比浊法测定患者术前1 d及术后第1、3天静脉血IgG、IgM和IgA水平。结果术后第1天，腹腔镜组和小切口组IgG、IgM和IgA均较术前明显降低，差异均有统计学意义（P＜0.05）；术后第3天腹腔镜组和小切口组IgG、IgM和IgA与术前基本一致，差异均无统计学意义（P＞0.05）；两组手术前后IgG、IgM和IgA水平比较，差异均无统计学意义（P＞0.05）。结论腹腔镜与小切口子宫肌瘤剥除术对体液免疫功能影响无显著差异。

【Abstract】Objective To study the effect of laparoscopic andminila parotomy tomyomectomy on the humoral immune function.Methods A totalof100 patients with uterinemyomas received laparoscopic myomectomy or minilaparotomy myomectomy in ourhospital from January 2013 to January 2015 were selected and randomly divided into the laparoscopic group andminila parotomy group，50 cases in each group.The levelsofvenous blood IgG，IgM and IgA on preoperative 1 d and postoperative 1，3 d were measured by the immunotur bidimetry respectively.Results The levels of IgG，IgM and IgA on postoperative 1 d in the two groupswere significantly decreased compared with before operation，the difference was statistically significant（P＜0.05）；the levels of venous blood IgG，IgM and IgA on postoperative 3 d in the laparoscopic group andminila parotomy group were basically consistentwith before operation，the difference was not statistically significant（P＞0.05）；the levelsof IgG，IgM and IgA in the two group shad no statistical difference between before and after operation（P＞0.05）.Conclusion The influence of laparoscopic and minilaparotomy myomectomy on thehumoral immune function hasno significant difference.

腹腔镜手术是一种微创手术，具有创伤小、出血少、感染率低和住院时间短等优点。腹腔镜子宫肌瘤剥除术（LM）由于具有创伤小、手术视野清晰、出血少及术后恢复快等优点而逐渐受到妇产科医生的重视［1］。近年来，研究LM对机体免疫功能的影响报道较多［2-3］，但目前鲜见将LM和小切口子宫肌瘤剥除术对患者体液免疫功能影响比较的报道。
