天麻钩藤饮联用波依定治疗高血压的疗效及机制研究

目的探讨天麻钩藤饮联用波依定治疗原发性高血压（简称高血压）的疗效及作用机制。方法选择2012年2月～2015年6月在我院诊治的高血压患者80例，随机分为治疗组与对照组各40例，对照组给予波依定治疗，治疗组给予天麻钩藤饮联用波依定治疗，两组都治疗1个月。结果治疗组的显效率和总有效率均明显高于对照组(P＜0.05)；两组治疗后的血压和血管紧张素Ⅱ(AngⅡ)都明显下降(P＜0.05)，但治疗组的降低幅度明显大于对照组(P＜0.05)；治疗组治疗期间的头痛、失眠、咳嗽、心悸等不良反应发生情况明显少于对照组(P＜0.05)。结论天麻钩藤饮联用波依定治疗高血压可有效提高疗效，减少波依定的不良反应，其作用机制可能与降低血浆AngⅡ表达有关。

[Abstract]Objective To discuss the curative effects of Tianma Gouteng decoction(TGD)combined with plendil on hypertension and itsmechanism.Methods 80 patientswith essential hypertension who received treatment in the hospital between February 2012 and June 2015 were selected and equally and random ly divided into treatment and control groups.The control group received plendil, and the treatment group received TGD combined with plendil.Both groups received onemonth of treatment.Results The excellence and the total effective rate of the treatment group were significantly higher than those of the control group(P＜0.05).After the treatment,the blood pressure and the angiotensinⅡ(AngⅡ)of both groups signficaintly decreased,but the decreasing extent of the treatment group was larger than that of the control group(P＜0.05).During the treatment,the incidence of adverse reactions like headache,insomnia,cough,and heart palpitation,etc in the treatmentgroup was significantly less than that in the control group(P＜0.05).Conclusion TGD combined with plendil for the treatment of hypertension can improve the curative effects and reduce the incidence of adverse reactions to plendil.Itsmechanism may be related to the decrease of plasma AngⅡexpression.

1　资料与方法

1.1病例资料选择2012年2月～2015年6月在我院诊治的80例高血压患者，均符合高血压及中医阴虚阳亢证的诊断标准，纳入标准：年龄20～70岁；知情同意本研究且得到医院伦理委员会的批准。排除标准：合并有肝、肾和造血系统等严重疾病；妊娠或哺乳期妇女；对治疗药物过敏；有继发性高血压病、心脏瓣膜病患者。根据患者入组的顺序编号，采用随机数字表法分为治疗组与对照组各40例，治疗组中，男21例，女19例；年龄20～69（52.12±4.19）岁；收缩压为（149.23±12.87）mmHg，舒张压为（97.33± 11.09）mmHg；合并疾病：糖尿病12例，冠心病11例；高血压分级：Ⅰ级18例，Ⅱ级12例，Ⅲ级10例。对照组中，男20例，女20例；年龄20～69（52.45±4.22）岁；收缩压为（149.15±12.34）mmHg，舒张压为（97.13± 11.56）mmHg；合并疾病：糖尿病11例，冠心病12例；……
