氨氯地平联合厄贝沙坦治疗糖尿病合并高血压疗效观察

目的研究氨氯地平联合厄贝沙坦对糖尿病合并高血压的疗效及对QT离散度的影响。方法选择2012年5月1日～2014年9月30日在本院就诊的2型糖尿病合并高血压患者120例为研究对象，随机分为3组各40例，对照A组给予厄贝沙坦治疗，对照B组给予苯磺酸氨氯地平治疗，观察组给予厄贝沙坦联合苯磺酸氨氯地平治疗。治疗两个月后比较3组血压水平、QT间期离散度及治疗效果。结果观察组显效率和总有效率均明显高于对照A组和B组（P＜0.05）；观察组24 h平均收缩压（24hSBP）、24 h平均舒张压（24hDBP）、24 h平均收缩压变异度（24hSBPV）、24 h平均舒张压变异度（24hDBPV）明显低于对照A组、B组（P＜0.05）；观察组的QT间期、校正QT间期、QT间期离散度均低于对照A组和对照B组(P＜0.05)。结论氨氯地平联合厄贝沙坦能够更为有效地降低糖尿病合并高血压患者血压水平和QT间期离散度，改善左心室肥厚，提高治疗效果。

[Abstract]Objective To study the curative effects of amlodipine combined with irbesartan on diabetic hypertension and the influence on QT dispersion.Methods 120 cases of type 2 diabetes combined with hypertension visited the hospital between May 1, 2012 and September 30,2014 wereenrolled as the research objects and random ly divided into three groupswith 40 ones in each group. Control group A received irbesartan,and control group B received am lodipine.Observation group received irbesartan combinedwith am lodipine.After 2 months of treatment,comparison wasmade in the blood pressure levels,QT dispersion ratio,and therapeutic effects among the three groups.Results The excellence and the total effective rate of the observation group were significantly higher than those of control group A and B(P＜0.05).The 24 h SBP,24 h DBP,24 h SBPV,and 24 h DBPV of the observation group were significantly lower than those of control group A and B(P＜0.05).The QT interval,corrected QT interval,QT interval dispersion ratio of the observation group were lower than those of the control group A and B(P＜0.05).Conclusion Amlodipine combined with irbesartan canmore effectively reduce blood pressure levels and QT interval dispersion of diabetic patientswith hypertension,improve left ventricular hypertrophy,and improve the therapeutic effects.

1　对象与方法

1.1研究对象选择2012年5月1日～2014年9月30日在本院就诊的2型糖尿病合并高血压患者120例进行研究，均符合高血压及2型糖尿病的诊断标准，报请医院伦理委员会批准，患者被告知研究事项后签署知情同意书。排除心绞痛、心肌梗死、继发性高血压、心力衰竭患者及对本研究所用药物过敏者。根据患者就诊先后顺序编号，采用依次入组的方法随机分为3组各40例。对照A组：男29例，女11例；年龄40～65（55.84±3.96）岁；身高162～178（165.36±2.14）cm；体重48～82（56.35±5.74）kg。对照B组：男26例，女14例；年龄40～68（55.39±3.47）岁；身高163～180（166.14±3.12）cm；体重50～80（55.68± 5.32）kg；……
