冠心病不稳定型心绞痛中医证候及其症状分布特征的研究*

目的探讨冠心病不稳定型心绞痛（冠心病UAP）的中医证候及其症状的分布特征。方法对411例冠心病UAP患者进行临床流行病学调查，用所得临床数据建立数据库，采用描述性统计（频数分布）和χ2检验等分析方法对临床所得数据库中的证候及症状分布进行统计分析。结果在411例患者中，主要证候是心血瘀阻证、气虚血瘀证、痰阻心脉证、阳气虚衰证；主要症状是胸痛、面部异常（面部萎黄、苍白、白、瘀暗、晦暗等）、胸闷、口唇紫绀。结论冠心病UAP的中医证候及其症状出现频次各有不同特征，与心关系最为密切，并且涉及五脏，对冠心病UAP患者的中医临床诊断具有极大指导意义。

【Abstract】Objective：To investigating the characteristic distribution of TCM syndromes and their symptoms in patients with coronary heart disease with unstable angina pectoris.M ethods：Clinical epidemiology of 411 cases with coronary heart disease（CHD）instability angina pectoris was investigated，and clinical data was used to establish a database and descriptive statistics（frequency distribution）andχ2test analysismethod of clinical derived from the database of symptoms and syndrome distribution were used for statistical analysis.Results：In 411 cases with unstable angina pectoris，themain syndromeswas blood stasis，Qi deficiency and blood stasis syndrome，syndrome of phlegm blocking heart vessel，Yang asthenia syndrome；main symptom was chest pain，abnormal facial （facial complexion，pale，yellow white，blood stasis dark，dull and so on），chest tightness，lip cyanosis.Conclusion：The frequency of TCM Syndromes and symptoms in coronary heart diseasewith unstable angina pectoris has various features，and heart ismost closely and related to the five internal organs，which has great guiding significance on unstable angina pectoris in clinical diagnosis of traditional Chinesemedicine.

冠心病是指冠状动脉粥样硬化使血管管腔狭窄，供血不足，导致心肌缺血缺氧引起的临床综合征，其临床症状以阵发性胸骨后压榨性疼痛为主，是一种常见的心血管疾病，且发病率有逐年上升的趋势［1］。冠心病不稳定型心绞痛（简称冠心病UAP）属于中医的“胸痹”“心痛”范畴，胸痹心痛是由多种原因引起的心脉痹阻不通，心之阴阳气血失调为主要病机［2］。本研究运用临床流行病学的方法，对411例冠心病UAP患者的中医证候及其症状分布特征进行了研究。……
