【摘要】 目的 探讨文拉法辛联合小剂量米氮平和阿立哌唑治疗老年抑郁伴失眠和焦虑患者的疗效。方法 56例老年抑郁症患者均给予文拉法辛联合小剂量米氮平和阿立哌唑治疗， 采用汉密尔顿抑郁量表（HAMD）和汉密尔顿焦虑量表（HAMA）分别在治疗前和治疗2、4、8周后评定疗效。结果 患者经治疗均取得了较好效果， 治疗2、4、8周后HAMD和HAMA减分显著， 均低于治疗前（P<0.01）。结论 文拉法辛与小剂量米氮平及阿立哌唑联合用药可快速改善老年抑郁伴失眠和焦虑患者的临床症状， 提高患者服药的依从性和战胜疾病的信心， 降低了苯二氮类药物使用量， 且不良反应无明显增加。

【Abstract】 Objective To investigate curative effect by venlafaxine combined with small dose of mirtazapine and aripiprazole in the treatment of senile depression complicated with insomnia and anxiety patients. Methods There were 56 senile depression patients， who received venlafaxine combined with small dose of mirtazapine and aripiprazole for treatment， receiving Hamilton depression scale （HAMD） and Hamilton anxiety scale （HAMA） for curative effect evaluation before and in 2， 4， 8 weeks after treatment. Results All patients had precise effect in treatment， and their HAMD and HAMA scores were all lower in 2， 4， 8 weeks after treatment than those before treatment （P<0.01）. Conclusion Combined drug therapy by venlafaxine combined with small dose of mirtazapine and aripiprazole can quickly improve clinical symptoms of senile depression complicated with insomnia and anxiety patients， along with compliance and confidence， and reduce applied dose of benzodiazepine， without any increased adverse reactions.

老年患者是抑郁症合并失眠、焦虑的高发群体， 无论是其本人还是亲属都希望病症能快速缓解， 同时又不过量或长期使用苯二氮类药物。作者探讨了文拉法辛联合小剂量米氮平和阿立哌唑治疗老年抑郁症伴失眠和焦虑的临床效果及安全性， 报告如下。

1 资料与方法

1. 1 一般资料 选取2012年8月～2013年12月门诊老年就医患者56例， 男26例， 女30例， 平均年龄（64.7±4.1）岁， 符合中国精神障碍分类与诊断标准（第3版）抑郁发作的诊断标准：HAMD 17项评分≥18分；HAMA评分≥14分；年龄≥60岁。排除标准：①严重躯体疾病；②双相情感障碍患者；③酒精和药物滥用史；④入组前未使用过任何抗抑郁剂和抗精神病药（但多数患者使用苯二氮类药物）。

1. 2 方法 所有入组患者均口服给予文拉法辛75 mg/早， 米氮平15 mg/晚， 阿立哌唑2.5 mg/晚；部分患者可合并用苯二氮类药（阿普唑仑0.8 mg/晚或艾司唑仑2 mg/晚）。入组后分别于治疗前和治疗2、4、8周后应用HAMD和HAMA评定疗效。

1. 3 统计学方法 采用SPSS11.0统计学软件进行统计分析。计量资料以均数±标准差（ x-±s）表示， 采用t检验。……
