中西医结合治疗不稳定型心绞痛疗效观察

［摘 要］目的：观察补阳还五汤加味联合硫酸氢氯吡格雷、阿托伐他汀钙治疗气虚血瘀型不稳定心绞痛（UA）的临床疗效和安全性。方法：125例随机分为两组，对照组64例用西医常规治疗，治疗组61例加用补阳还五汤加味治疗。结果：两组发作时间及发作次数与治疗前比较差异均有统计学意义（P＜0．05），治疗后两组比较差异有统计学意义（P＜0．05）；两组血清hs-CRP表达水平、血浆黏度、纤维蛋白原含量、胆固醇及甘油三酯水平与治疗前比较差异有统计学意义（P＜0．05），治疗后两组比较差异有统计学意义（P＜0．05）；总有效率治疗组90．16%、对照组73．44%，两组比较具有统计学意义（P＜0．05）。结论：补阳还五汤加味联合西药治疗UA疗效确切，能够稳定斑块、抑制炎症反应、调脂、改善血管内皮功能及血液黏稠度，明显缓解临床症状，安全可靠。

［Abstract］Objective：To observe the clinical effect and safety of in the treatment of unstable angina pectoris （UA） of Qi deficiency and blood stasis type. Method： 125 cases of UA were divided into two groups randomly. 64 cases were treated with clopidogrel combined with atorvastatin calcium as the control group while 61 cases were treated with Buyang Huanwu Decoction combined with clopidogrel，atorvastatin calcium as the treatment group. Result：There was statistical significance in the time and the number of attacks of both groups between post-treatment and pre-treatment（P＜0．05）.After the treatment， there was statistical significance between the two groups（P＜0．05）. There was statistical significance in the expression level of hs-CRP，plasma viscosity，fibrinogen content，the level of cholesterol and triglyceride in serum of both groups between post-treatment and pre-treatment（P＜0．05）. After the treatment， there was statistical significance between the two groups（P＜0．05）.The total effective rate of the treatment group was 91.16% while that of the control group was 73.44%. The total effective rate of the treatment group was superior to that of the control group（P＜0．05）. Conclusion：Buyang Huanwu Decoction combined with clopidogrel，atorvastatin calcium in the treatment of UA is effective，which could stabilize the plaque，inhibit inflammatory reaction，regulate blood lipid，improve vascular endothelial function and plasma viscosity and relieve clinical symptoms obviously， safe and reliable.

不稳定型心绞痛（unstable angina，UA）是由于冠脉供血和心肌耗血之间的平衡紊乱而导致的短暂急剧心肌缺血缺氧的临床综合征，是急性冠脉综合征（acutecoronary syndromes，ACS）的表现类型之一。近年来随着我国人群饮食习惯的改变，高血脂﹑高血糖﹑高血压等患者人数显著增加，本病患病率日益升高，且发病年龄有逐年下降趋势，成为威胁人类健康的常见心脏疾病之一［1］。因此，早期采取合理的治疗手段进行干预对UA造成的死亡率及改善患者临床预后具有重要临床意义。……
