[摘要] 目的 探讨赖脯胰岛素联合甘精胰岛素强化治疗糖尿病结核患者的临床疗效。 方法 选取2012年3月～2014年2月本院收治的60例糖尿病结核患者作为研究对象，将其随机分为治疗组和对照组，每组30例。对照组给予普通胰岛素降糖治疗；治疗组给予赖脯胰岛素联合甘精胰岛素强化治疗，比较两组患者痰菌阴转、病灶吸收、血糖控制、达标时间等情况。 结果 治疗1个月后，治疗组痰菌阴转率为68.2%（15/22），高于对照组的38.1%（8/21）（P<0.01）；治疗2个月后，治疗组痰菌阴转率为90.9%（20/22），高于对照组的76.2%（16/21）（P<0.01）。1个月后治疗组病灶吸收率为63.3%（19/30），高于对照组的30.0%（9/30）；治疗2个月后，治疗组病灶吸收率为90.0%（27/30），明显高于对照组的70.0%（21/30）（P<0.05）。治疗组血糖达标率为93.3%（28/30），高于对照组的66.7%（20/30）（P<0.05）；治疗组低血糖发生率低于对照组（P<0.05）。 结论 赖脯胰岛素联合甘精胰岛素强化治疗糖尿病结核能快速控制血糖，促进痰菌阴转及病灶吸收，安全有效。

Effect analysis on insulin lispro combined with insulin glargine in intensive treatment of diabetes mellitus and tubercular

[Abstract] Objective To discuss the clinical effects of insulin lispro combined with insulin glargine in intensive treatment of diabetes mellitus and tubercular. Methods 60 patients with diabetes mellitus and tubercular treated in our hospital from March 2012 to February 2014 were selected and randomly divided into the treatment group and the control group，with 30 cases in each group.Patients in the control group were given hypoglycemic therapy using regular insulin and patients in the treatment group were given intensive therapy by insulin lispro combined with insulin glargine.The sputum negative conversion rates，absorbing rates of tuberculous focus，blood glucose control and standard-reaching time between the two groups were compared. Results After one month of treatment，the sputum negative conversion rate of the treatment group was 68.2%（15/22），which was higher than the 38.1%（8/21） of the control group（P<0.01）.After two months of treatment，the sputum negative conversion rate of the treatment group was 90.9%（20/22），which was higher than the 76.2% （16/21） of the control group（P<0.01）.The absorbing rate of tuberculous focus of the treatment group after one month was 63.3%（19/30），higher than the 30.0%（9/30） of the control group；and after two months was 90.0%（27/30），significantly higher than the 70.0%（21/30） of the control group（P<0.05）.The blood glucose standard-reaching rate was 93.3%（28/30） in the treatment group，higher than the 66.7% （20/30） in the control group（P<0.05）.The incidence of hypoglycemia was lower in the treatment group（P<0.05）. Conclusion Combination of insulin lispro and insulin glargine can safely and effectively control the blood glucose and promote the negative conversion of sputum and absorbing of tuberculous focus.

糖尿病和结核病是两种严重危害人类健康的全球性疾病，在临床上较常见。全球范围内两病的发病率和患病率呈逐年上升趋势，两病合并人数也逐年增多。糖尿病合并肺结核的严重程度及复发率较单纯肺结核患者高[1]。治疗上肺结核的疗效和血糖密切相关[2]。糖尿病强化治疗能控制血糖、防止及延缓并发症，而含赖脯胰岛素强化治疗患者降低餐后血糖效果好，低血糖发生率低，降糖作用稳定[3-4]。本研究对我院近年应用赖脯胰岛素联合甘精胰岛素强化治疗糖尿病结核患者的近期临床疗效进行回顾性分析，再次明确良好的血糖控制可加速结核病的愈合，取得满意疗效。
