硫酸镁在围手术期应用的研究进展

【摘要】硫酸镁为生理性镁离子补充剂、天然的非竞争性N-甲基-D-天冬氨酸受体阻滞剂以及钙通道阻滞剂。在围手术期应用硫酸镁能够抑制应激反应、辅助镇痛、抗炎，其还具有抗苏醒期躁动、治疗术后心律失常等作用。该文就硫酸镁在围手术期的临床应用及研究进展进行综述，主要介绍了硫酸镁在围手术期对心血管系统、呼吸系统、中枢神经系统、免疫系统的影响及其在麻醉镇痛中的作用。

Abstract【】Magnesium sulfate is capable of not only supplementing physiological magnesium ions but also functioning as a natural, noncompetitive N-methyl-D-aspartate receptor (NMDA) blocker and a calcium channel blocker. The perioperative application of magnesium sulfate can suppress stress responses, reduce pain, and inhibit inflammatory reactions. Studies have also demonstrated that magnesium sulfate has therapeutic effects on postoperative arrhythmia and restlessness during the anesthesia recovery period. This article reviewed the progress of research addressing the clinical application of magnesium sulfate during the perioperative period, focusing on the effects of magnesium sulfate on the cardiovascular system, respiratory system, central nervous system and immune system, as well as the role of magnesium sulfate in anesthesia and analgesia.

镁离子是人体内不可缺少的离子之一，是细胞内除钾离子以外最多的阳离子，同时是体内新陈代谢过程中300多种酶活动的辅助因子。镁能维持核酸结构的稳定性，参与蛋白质与核酸的合成、肌肉收缩等多种生理过程。硫酸镁不仅在哮喘、高血压危象、先兆子痫等疾病的防治中发挥重要作用，而且能够维护患者在围手术期的生命安全。本文就硫酸镁对机体各系统及麻醉镇痛的相关作用及应用进展进行阐述。

一、硫酸镁对机体各系统的影响

1.心血管系统

1.1预防气管插管时的心血管不良反应

全身麻醉患者在进行喉镜置入和气管插管操作时，机体产生强烈的应激反应，特别是有基础高血压病病史的患者，更易发生心血管的不良反应，如颅内出血、心肌缺血、心律失常等。硫酸镁作为钙离子拮抗剂，可减少儿茶酚胺类神经递质的释放，抑制应激反应，维持血流动力学平稳。……
