【摘 要】 目的 探讨老年患者髋部骨折的临床治疗方法。方法 回顾性分析80例老年髋部骨折患者的临床资料。80例中治疗前有内科合并症的46例积极治疗其并发症；手术治疗75例，保守治疗5例。结果 75例手术患者均安全度过麻醉、手术期；术后并发症：精神障碍8例，肺部感染2例，肺栓塞1例，深静脉血栓2例，死于肺栓塞1例；79例康复出院，平均住院时间18d。5例给予患肢皮牵引的保守治疗患者均出现肺部感染或褥疮，出院后1年内均死于相关并发症。结论 老年患者髋部骨折能耐受手术的应及早行手术治疗，选择合适的手术方式、充分的术前准备、积极正确地处理并发症是改善老年髋部骨折患者生活质量的关键。

【Abstract】 Objective To investigate the clinical therapy for elderly patients with hip fracture. Methods A retrospective analysis of the clinical data of 80 cases of senile patients with hip fracture. In 80 cases there are 46 cases before treatment department of internal medicine complications actively cure its complications；surgical treatment of 75 cases， 5 cases of conservative treatment. Results 75 cases of patients were safe through the anesthesia， operation； postoperative complications： 8 cases of mental disorders， 2 cases of pulmonary infection， 1 cases of pulmonary embolism， 2 cases of deep vein thrombosis， 1 cases died of pulmonary embolism； 79 cases discharged， average hospitalization time of 18 D. 5 cases were treated with conservative treatment in patients with limb skin traction were pulmonary infection or bedsore， died of complications related to 1 years after discharge.Conclusion Elderly patients with hip fracture， who can be tolerated and early surgical operation treatment ， the choice of surgical approach and appropriate sufficient preoperative preparation， active treatment of complications is correctly to improve the life quality.

髋部骨折已经成为世界范围内危害老年人生活质量及致死的重要原因之一[1]。由于老年人身体各器官功能减退，且常并存多种疾病，伤后长期卧床易致肺部感染，泌尿系结石，褥疮等并发症；但手术风险较大，手术治疗的必要性及其方式尚存争议， 临床应在客观评估后选择合适的治疗方案。2010年5月至2012年5月本院共收治80例老年髋部骨折患者，现将其临床资料回顾分析如下。

1 资料与方法

1.1 一般资料

80例髋部骨折患者：男33例，女47例；年龄60～96岁，平均76.5岁；股骨颈骨折41例（头下型11例，经颈型20例，基底型10例），股骨粗隆间骨折39例，其中合并股骨头坏死2例。合并糖尿病13例，慢性支气管炎20，高血压病患者25（其中部分患者上述2～3种内科病并存）。……
