单纯性PT或APTT延长临床分析

摘要: 目的:探讨单纯性凝血酶原时间(PT)或活化部分凝血活酶时间(APTT)延长的临床价值。 方法: 采用LG-PABER-I型血小板集聚凝血因子分析仪用一期法检测PT,凝固法检测APTT,若PT或APTT延长则用肝素酶和正常人血浆作纠正试验,同时测定凝血因子活性度。结果:单纯性PT延长189例,其中标本不合格18例(9.52%), 先天性凝血因子功能障碍47例(24.87%),獲得性凝血因子功能障碍124例(65.61%);单纯性APTT延长252例,其中标本不合格34例(13.49%), 先天性凝血因子功能障碍142例(56.35%), 获得性凝血因子功能障碍76例(30.16%)。 结论: PT或APTT延长的影响因素包括先天性和获得性凝血因子功能障碍以及不合格标本等,PT延长主要反映外源性因素,APTT延长主要反映内源性因素。

Abstract:Objective:To explore the clinical value of simple TP or APTT extension. Method:Based on LG-PABER-I blood platelet coagulation factor analyzer, PT was detected by one-stage method; APTT was detected by freezing method. If PT or APTT was extended, heparinase and normal human plasma was applied. The activity of blood coagulation factor was detected. Result: PT for 189 cases was extended of which there were 18 unqualified specimens (9.52%), 47 cases with congenital dysfunction of coagulation factor (24.87%) and 124 cases with acquired dysfunction of coagulation factor (65.61%). APTT for 252 cases was extended of which there were 34 unqualified specimens (13.49%), 142 cases with congenital dysfunction of coagulation factor (56.35%) and 76 cases with acquired dysfunction of coagulation factor (30.16%). Conclusion: The influence factor for PT or APTT extension includes the congenital and acquired dysfunction of coagulation factor as well as unqualified specimen. PT extension is majorly reflected in the exogenous aspects while APTT extension is majorly reflected in the endogenous aspects.

凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)两项检测是判断机体止血与凝血系统病理变化的重要指标,是临床上最常用、最基本的凝血系统的筛选试验,也是临床溶栓、抗凝治疗及手术前的必要检查项目,其测定结果可反映体内外源性和内源性凝血途径以及凝血共同途径的状况[1]。在临床上常见单纯性PT或APTT延长的检测结果,而病人出血程度不一,甚至相当部分无出血倾向,对于此种情况临床医师难于判断准确原因,处理起来很棘手。为更好指导临床医师作出准确判断,自2009年1月至2014年5月对我院血液实验室凝血功能检测结果进行了综合分析,情况如下:
1、临床资料与方法
1.1一般资料 2009年1月至2014年5月在我院血液实验室检测凝血功能异常标本共6722例,选择单纯性PT或APTT延长(PT超过正常对照3秒以上,APTT超过正常对照10秒以上)者作为研究对象。……
