[摘要] 目的 通过观察分析常规药物治疗联合地塞米松加利多卡因局部封闭治疗对疼痛与肿胀的缓解作用，探讨治疗根管治疗期间急症方法。方法 选取该院2011年1月—2012年1月进行根管预备后发生急性根尖疼痛的80例患牙，随机均分为实验组与对照组，对照组40例患牙给予头孢氨苄片和甲硝唑片口服；实验组40例患牙给予头孢氨苄片和甲硝唑片口服，并行根尖区地塞米松加利多卡因局部封闭治疗。对比两组间疼痛缓解率和根尖区肿胀发生率进行统计学处理。结果 实验组疼痛缓解率（75%）高于对照组（35%），实验组患者肿胀发生率（10%）低于对照组（30%）。两组患牙治疗效果差异具有统计学意义（P<0.05）。 结论 地塞米松加利多卡因局部封闭可以缓解根管预备后的疼痛，降低发展成根尖周脓肿的发生率，值得临床推广应用。

[Abstract] Objective To observe and analyze the role of conventional medication to ease joint dexamethasone plus lidocaine partial closure of pain and swelling，and to explore during the acute treatment of root canal treatment methods. Methods After root canal preparation will be pain 80 cases of acute apical teeth were randomly divided into two groups，were named as the experimental group and control group，the control group of 40 patients teeth give cephalexin tablets and metronidazole tablets oral；experimental group of 40 patients given teeth to give cephalexin tablets and oral metronidazole tablets， parallel apical dexamethasone plus lidocaine treatment of partial closure. Comparison between the two groups of pain response rate and the incidence of apical swelling，and statistical analysis. Results Experimental pain remission rate （75%） than the control group （35%），the incidence of swelling in the experimental group patients （10%）， lower than the control group （30%）.Two groups were comparable with dental treatment effect differences （P<0.05）. Conclusion Dexamethasone Gary lidocaine local closed can relieve pain after root canal preparation，reduce the incidence of development into periapical abscess， worthy of clinical popularization and application.

根管治疗期间很容易发生约诊间疼痛，严重者发生肿胀。尤其更容易发生在根管预备完成之后。持续疼痛不缓解，延长了治疗时间。虽有研究表明发生约诊间疼痛不会影响根管治疗的预后，但是增加了患者心理负担，降低了后续治疗信心和对医生的信任度，也易引起医患纠纷。该次研究对该院2011年1月—2012年1月期间治疗的80例根管预备后急性根尖疼痛患牙作为研究对象，观察常规药物治疗联合地塞米松加利多卡因局部封闭治疗的治疗效果，探索治疗根管治疗期间急性症状的有效方式，为临床提供有效治疗经验，现报道如下。

1 资料与方法

1.1 一般资料

在该院就诊进行根管预备当日和次日发生疼痛的病人80颗患牙。……
