桂林地区2262例女性生殖道HPV感染及基因型结果分析

[摘要]目的 了解桂林地区高危型人乳头状瘤病毒(HPV)感染及亚型分布情况,为预防子宫颈癌前病变、子宫颈癌提供参考依据。方法 采用医用核酸分子快速导流杂交基因芯片技术对2 262例妇女进行HPV-DNA基因结果分析。结果 2 262例妇女中检测出HPV阳性率26.22%,其中高危型感染率较高(19.20%),低危型感染率较低(7.02%);普通女性中的感染率为18.89%,在宫颈病变患者中的感染率为84.38%,两者相比差异有统计学意义(c2= 457.01,P=0.00<0.05);不同年龄段HPV阳性检出率不同,呈双峰U形曲线,第l峰位于年轻妇女(20~30 岁),阳性率为24.61%,第2峰位于中老年妇女(51~65岁),阳性率最高51.74%;高危型主要以HPV52、16、58为主。低危型以HPV6、11最为常见。结论 桂林地区2262例女性宫颈HPV感染常见高危型亚型是HPV52、16、58。低危型亚型是HPV6、11。感染年龄趋于年轻化和老年化。桂林地区的HPV感染基因分型符合亚洲人群分布规侓,但有其地区特点。

[Abstract]Objective Know guilin area type high-risk human papilloma virus (HPV) infection and subtype distribution, in order to prevent cervical cancer lesion before, and provides some references for the cervical cancer.Methods the medical nucleic acids rapid diversion hybrid gene chip technology on 2262 women in HPV DNA genetic results analysis.Results Detect the HPV positive rate of 26.22%, in 2262 women with high-risk type higher infection rate (19.20%), low-risk type low infection rate (7.02%); Ordinary women in the infection rate was 18.89%, the infection rate was 84.38% in patients with cervical lesions, compared the difference was statistically significant (c2 = 457.01, P= 0.00 < 0.05); Positive rate of HPV was different, different ages shows bimodal u-shaped curve, the first peak in young women l(20~30 years old), positive rate was 24.61%, the second peak is located in the middle-aged and old women (51~ 65 years), the highest positive rate of 51.74%; High-risk type mainly HPV52,16, 58. The most common low-risk type with HPV6 and 11.Conclusion Guilin area 2262 women type cervical HPV infection common high-risk subtype is HPV52, 16, 58. Low-risk type subtype is HPV6, 11. Infection tend to be younger and older age. Genotyping of HPV infection in guilin are in Asian population distribution rules denied, but has its regional characteristics.

基础研究和流行病学均已证实感染人乳头状瘤病毒(human papillomavirus,HPV)是造成宫颈上皮内瘤变(Cervical Intraepithelial Neoplasia,CIN)以及宫颈癌的重要原因[1]。目前已经有200余种HPV得到了鉴定,其中有30多种亚型会导致宫颈病变[2]。感染不同亚型HPV由于分布的地域性差异,所产生的致癌性和后果也不同[3]。经过世界范围内的研究,由于种族和地理人群的差异,获得的HPV流行状况、感染峰龄和病毒类型也不尽相同[4-5]。该研究通过医用核酸分子快速导流杂交基因芯片技术,对2012年7月—2014年11月间该区感染HPV妇女宫颈和基因亚型的分布进行探析,基本对该地区妇女宫颈感染HPV情况有所了解,为该地区防治妇女宫颈癌、研发疫苗和使用提供依据。现报道如下。endprint
1 资料与方法
1.1 一般资料
该研究对桂林市妇女儿童医院妇科门诊就诊及体检共计2262例妇女进行HPV检测。行宫颈液基薄层细胞学(Thinprep Cytology Test,TCT)检测对检测结果阳性者。若HPV结果阳性合并TCT检查异常者要进行阴道镜检查同时在病变处取活检,通过病理检查确诊。……
