[摘要]目的 探讨脂血对Sysmex XT-1800i检测血液分析指标的影响。 方法 取无溶血、脂血和黄疸的全血标本40例，每例标本分装成4管，每例第1管组成对照组（n=40），其余3管分别添加脂肪乳剂制成低水平脂血标本组（n=40）、中水平脂血（n=40）和高水平脂血（n=40）。用Sysmex XT-1800i测定各组血液分析指标。结果 低水平脂血组 血标本WBC、RBC、Hb、MCV、MCH、MCHC、PLT测定结果与对照组比较，差异无统计学意义（P>0.05）。中水平脂血组Hb、MCH、MCHC测定结果与对照组比较，差异有统计学意义（P<0.05）。高水平脂血组Hb、MCV、MCH、MCHC测定结果与对照组比较，差异有统计学意义（P<0.05）。结论 中度或重度脂血可影响Sysmex XT-1800i检测血液分析结果。

The Effect of Lipohemia on the Detection Results of Blood Analysis by Sysmex XT-1800i

[Abstract] Objective To discuss the effect of lipohemia on the detection results of blood analysis by Sysmex XT-1800i. Methods 40 cases of blood samples were selected without hemolysis， lipohemia and jaundice. Each case was packed into 4 tubes. The first tube of each case was used for control group（n=40）， and the remaining 3 tubes are respectively added by fat emulsion and prepared for low level of lipohemia samples group（n=40）， the middle level of lipohemia（n=40） and high the level of lipohemia （n=40）. Blood analysis markers in each group were detected by Sysmex XT-1800i. Results The values of RBC， Hb， MCV， WBC， MCH， MCHC， PLT showed no significant differences in low level of lipohemia group when compared to the control（P>0.05）. The values of Hb， MCH， MCHC showed significant differences in middle level of lipohemia group when compared to the control（P<0.05）. The values of Hb， MCV，MCH， MCHC showed significant differences in high level of lipohemia group when compared to the control（P<0.05）. Conclusion Middle or high level of lipohemia has an effect on the detection results of blood analysis by Sysmex XT-1800i.

Sysmex XT-1800i 是一台先进的全自动血液分析仪，采用半导体流式细胞分析系统、结合细胞化学染色技术、荧光检测技术，性能稳定，在临床检验中得到了广泛应用[1]。由于血液成分复杂，溶血和脂血因素对血清生化指标的影响较为明确，但对血液分析指标的影响罕见报道[2-3]。溶血对血液分析的影响报道较多，但脂血对血液分析指标的影响却较少报道[4-5]。该研究选取该院2014年11月—12月健康体检人群的EDTA抗凝全血标本40例作为研究对象，探讨脂血因素对Sysmex XT-1800i检测血液分析指标的影响。现报道如下。

1 资料与方法

1.1 一般资料

选择该院2014年11月—12月健康体检人群的EDTA抗凝全血标本40例，所有标本无溶血、脂血和黄疸。研究对象年龄分布在19～45岁之间，其中男性20例，女性20例。

1.2 仪器、试剂和方法

1.2.1 仪器和试剂 Sysmex XT-1800i及血液分析检测试剂购自Sysmex公司；日立7080全自动生化分析仪购自日立公司；TG检测试剂购自四川迈克生物诊断有限公司。脂肪乳剂购自德国 B.Braun 公司。EDTA-K2真空采血管购自BD公司。
