[摘要] 目的 分析和探讨骨外科临床上对于老年膝关节退行性骨关节炎患者采用关节置换进行治疗的临床效果，以期为该疾病患者的临床治疗提供借鉴。方法 对2010年7月—2013年10月该院骨外科收治的30例老年膝关节退行性骨关节炎患者临床资料进行回顾性分析，所有患者进行为期1年的术后随访，观察和比较患者术前、术后1、6、12个月关节的活动度、疼痛度，对其进行HSS 评分，测定患者四头肌等长收缩的最大肌力以及随意运动肌力，同时测量患者治疗前后两侧股四头肌的肌力。 结果 经过手术治疗患者术后1、6个月的关节活动度、疼痛度以及HSS 评分显著高于手术前（P<0.05），术后第12个月的关节活动度、疼痛度以及HSS 评分与手术前相比差异有统计学意义（P<0.01）；经过治疗患者的四头肌等长收缩最大肌力显著优于治疗前（P<0.05）；在随意运动肌力方法治疗前后差异无统计学意义（P>0.05）。 结论 该研究结果表明，骨外科临床上对于老年膝关节退行性骨关节炎患者采用关节置换进行治疗具有良好的临床效果，该方法能显著改善患者术后的关节功能恢复，提高患者的生存和生活质量，因此值得在骨外科临床上推广使用。

Joint replacement treatment of 30 cases of senile degenerative osteoarthritis of knee joint clinical reports

[Abstract] Objective To analysis and discussion on bone surgery for elderly patients with knee degenerative osteoarthritis clinically the joint replacement in the treatment of the clinical effect of so as to provide reference for clinical treatment of patients with the disease. Methods 30 cases of bone surgery treated in our hospital clinical data in elderly patients with degenerative osteoarthritis of knee joint were analyzed retrospectively， all patients postoperative follow-up for 1 year， observed and compared with preoperative and postoperative 1， 6， 12 months joints， pain degree of mobility， HSS score， determination of quadriceps in patients with isometric contraction of maximum strength and voluntary movement strength， at the same time measurement on both sides before and after treatment in patients with femoral quadriceps muscle strength. Results After 1， 6 months postoperatively in patients with surgical treatment of joint motion， pain degree and HSS score significantly higher than that before surgery（P<0.05）， the motion of 12 months after surgery， pain degree and HSS score extremely significant difference compared with before operation （P< 0.01）； After treatment in patients with quadriceps isometric contraction biggest strength significantly better than before treatment （P<0.05）； In a casual sports strength method before and after treatment there was no significant difference （P>0.05）. Conclusion This study results show that the bone surgery for elderly patients with knee degenerative osteoarthritis clinically with joint replacement therapy has good clinical effect， this method can significantly improve patients with postoperative recovery of joint function， improve the patients' survival and quality of life， thus is worth popularizing in orthopedic surgery clinical use.
