2015209 Correlation between functional dyspepsiaoverlapped irritable bowel syndrome and gastrointestinal hormones in the elderly.CHEN Lei(陈磊),et al.Clin Med Coll,Yangzhou 225001.Chin J Geriatr 2015;34(4):394-396.

Objective To investigate the relationship between functional dyspepsia-overlapped irritable bowel syndrome and gastrointestinal hormones in the elderly.Methods Totally 60 cases with functional dyspepsia-overlapped irritable bowel syndrome were collected and divided into elderly group and non-elderly group(n=30 each).20 healthy elderly people were considered as control group. Levels of serotonin,somatostatin,gallbladder angiotensin,calcitonin gene-related peptide,vasoactive intestinal peptide,substance P were detected.Results Compared with non-elderly patients,elderly patients showed that levels of cholecystokinin,serotonin,and vasoactive intestinal peptide were increased(all P<0.05),and somatostatin level was reduced(P<0.05).Compared with control group,levels of cholecystokinin,calcitonin generelated peptide,serotonin,vasoactive intestinal peptide and substance P were increased(all P<0.05)in elderly patients.ConclusionTheincidenceoffunctional dyspepsia-overlapped irritable bowel syndrome is lower in the elderly than in the non-elderly.The functional dyspepsia-overlapped irritable bowel syndrome has a close correlation with gastrointestinal hormones in the elderly.

2015210 An analysis for the clinical difference between post infectious irritable bowel syndrome and non post infectious irritable bowel syndrome.SHI Hui(石卉),et al.Dept Gastroenterol,PLA General Hosp,Beijing 100853.Chin J Intern Med 2015;54(4):326-329.

Objective To study the clinical discrepancy between patients with post infectious irritable bowel syndrome(PIIBS)and non post infectious irritable bowel syndrome (NPI-IBS),and assess the value of serum intestinal fatty acid binding protein(I-FABP)for differential diagnosis. Methods A total of 117 patients with PI-IBS,201 patients with NPI-IBS and 31 healthy controls were prospectively recruited in General Liberation Army Hospital from 2010 to 2013.Plasma samples and clinical data were collected.Serum I-FABP level was measured by an enzyme-linked immunosorbent assay.Results The median age of patients with PI-IBS was 36 years.The median time of diagnosis in PI-IBS group was significantly longer than that in NPI-IBS group[(19.7±10.3)months vs (11.4±5.3)months,P<0.05].Similarly,the proportion of anxiety[58.1%(68/117)vs 28.9(58/201),P<0.05]and the value of I-FABP[(42.6±14.8)μg/ L vs(17.3±11.5)μg/L,P<0.05]in PI-IBS group were significant higher than NPI-IBS patients.The level of I-FABP of healthy controls[(10.6±8.2)μg/L]was also significantly lower than that of PI-IBS patients(P<0.05),yet no difference from that of NPI-IBS group. The I-FABP value of subgroup PI-IBS patients with diarrhoea(IBS-D)was significant higher than that of NPIIBS group[(54.8±9.3)μg/L vs(12.3±6.2)μg/ L,P<0.05].However,other parameters including gender,age,GSRS score,and I-FABP value of subgroup constipation(IBS-C)and mix(IBS-M),were not different between PI-IBS group and NPI-IBS group(all P>0.05).Conclusion PI-IBS is an occult intestinal inflammation disease with mucosa injury.I-FABP might be a potential testing marker for the diagnosis of PI-IBS.

2015211 Comparison of the predictive value of the sixth and seventh edition TNM pT classificati ons inprognosis for gastric cancer.ZHANG Peng(张鹏),et al.Dept Surg,201st Hosp,PLA,Liaoyang 111000. Chin J Oncol 2015;37(3):190-194.

Objective To analyze the clinical application value of the 7th edition UICC-AJCC TNM pT classification of gastric cancer.Methods The classification of tumor invasion depth of 874 cases of gastric cancer was determined using the 7th edition UICC-AJCC TNM classification and the relationships of prognosis with clinicopathological factors was compared with that obtained using the 6th edition TNM classification.Results The 5-year survival rates according to the 6th edition pT1,pT2,pT3,pT4 were 98.4%,55.8%,26.0%and 24.%,respectively,showing a significant difference among the four substages(P<0.001).The 5-year survival rates according to the 7th edition pT1a,pT1b,pT2,pT3,pT4a,and pT4b were 98.6%,98.2%,75.8%,48.9%,26.0% and 24.5%,respectively,and there were significant differences in survival time among the six substages(P<0.001).According to the 7th edition TNM classification,for the pT2 and pT3 patients groups,the 5-year survival rates of the 7th edition pN0(without lymphatic metastasis)were 84.8%and 77.9%,respectively,showing no significant difference between the two survival rates(P=0.204).The 5-year survival rates according to the 7th edition pNx(with lymphatic metastasis)were 66.9%and 37.5%,respectively,and there was a significant difference between the two survival times(P<0.001).In multivariate analysis,the 7th edition pT stage,vascular or lymphatic invasion,the 6th edition pT staging and primary tumor site were independent prognostic factors in patients with gastric cancer(P<0.05),and the relationship of the staging of the 7th version of pT was more close to the prognosis of patients with gastric carcinoma than that of the 6th edition of pT staging system.Conclusion The 7th edition UICC-AJCC classification of pT staging of gastric cancer patients has a better prognostic predictive value than the 6th edition.

2015212 Impact of glutamine,eicosapntemacnioc acid,branched-chain amino acid supplements on nutritional status and treatment compliance of esophageal cancer patients on concurrent chemoradiotherapy and gastric cancer patients on chemotherapy. Objective To explore the effects of glutamine,eicosapntemacnioc acid(EPA)and branched-chain amino acids supplements in esophageal cancer patients on concurrent chemoradiotherapy and gastric cancer patients on chemotherapy.Methods From April 2013 to April 2014,a total of 104 esophageal and gastric carcinoma patients on chemotherapy or concurrent chemoradiotherapy were recruited and randomly divided into experimental and control groups.Both groups received dietary counseling and routine nutritional supports while only experimental group received supplements of glutamine(20 g/d),EPA (3.3 g/d)and branched-chain amino acids(8 g/d). And body compositions,blood indicators,incidence of complications and completion rates of therapy were compared between two groups.Results After treatment,free fat mass and muscle weight increased significantly in experiment group while decreased in control group(P<0.05).And albumin,red blood cell count,white blood cell count and blood platelet count remained stable in experiment group while declined significantly in control group.During treatment,compared to control group,the incidences of infection-associated complication were lower(6%vs 19%,P<0.05)and the completion rates of therapy were significantly higher in experiment group (96%vs 83%,P<0.05).Conclusion Supplements of glutamine,EPA and branched-chain amino acids can help maintain nutrition status,decrease the complications and improve compliance for esophageal cancer patients on concurrent chemo-radiotherapy and gastric cancer patients on postoperative adjuvant chemotherapy.

Objective To study the effect of platelet derived growth factor-β and its receptor expression on the proliferation of renal cell carcinoma ACHN cells in vitro and in vivo.Methods PDGF-B gene was transfected into human renal carcinoma cell line ACHN cells,and the proliferation capability of ACHN cells transfected with or without PDGF-B was assessed by MTT assay.The effect of PDGF-B on the expression of pPDGFR-β in endothelial cells and vascular smooth muscle cells(VSMC)was detected by Westem blot.ACHN cells transfected with PDGF-B were injected into mice(untransfected ACHN as control)to induce tumor formation.Immunohistochemical staining was used to detect the expression of Ki-67 in tumor cells and the tumor volume was measured to compare the tumor growth in the two groups.Results The PDGF-B expression of ACHN cells in transfected group was significantly increased than that in the untransfected group.MTT assay showed that the proliferation capability of ACHN cells in the transfected and untransfected groups had no significant differences at different time points(P>0.05).The expression of p-PDGFR-β in VSMC was significantly increased when cultured with PDGF-B overexpressionculturemedium.Themean tumor size of the PDGF-B group and control group was (0.305±0.108)cm3and(0.577±0.218)cm3,respectively(P=0.007).Ki-67-positive tumor cells were (41.00±5.34)/HPF in the PDGF-B-transfected group and(55.80±2.95)/HPF in the untransfected group (P=0.001).Conclusion PDGF-B overexpression may up-regulate p-PDGFR-β expression of VSMC in renal cell carcinoma,and inhibit the tumor cell proliferation and tumor growth through paracrine signaling.

Objective To investigate the effects of heat shock protein 70(Hsp70)inhibitor 2-phenylethynesulfonamide (PES)on viability,migration,and nuclear factor-κB (NF-κB)signal pathway of human gastric cancer BGC cells.Methods The viability and migration of human gastric cancer BGC cells after treatment with PES were detected by cell counting kit-8(CCK-8)and wound healing experiments,respectively.The activity of NF-κB signal pathway and the expression of vascular endothelial growth factor(VEGF)protein of BGC cells after treatment with PES were measured by Western blotting.The expression of p65 in cytoplasm of BGC cells after treatment with PES was determined by immunofluorescence. Results As compared with BGC cells without treatment with PES(as a control),the viability and migration ability of BGC cells after treatment with 5,10 and 20 μmol/ L PES for 72 h were significantly inhibited(both P<0.05).The expression levels of p65,phospho-p65 (p-p65), inhibitoryproteinofNF-κBkinasea (IKKα),p-IKKα,p-IKKβ,IKKβ,p-Akt and Akt in NF-κB signal pathway and VEGF protein of BGC cells after treatment with 20 μmol/L PES were lower than those of the control cells(all P<0.05).The fluorescence expression of p65 in cytoplasm of BGC cells after treatment with 20 μmol/L PES was obviously decreased. Conclusion Hsp70 inhibitor PES can inhibit the proliferation and migration of BGC cells,down-regulate the activity of NF-κB signal pathway and the expression level of VEGF protein,and decrease the expression of p65 in cytoplasm of BGC cells.

Objective To investigate the effect and mechanism of Vav3 gene on the proliferation of human gastric cancer cell line SGC7901.Methods The expressions of Vav3 protein in gastric cancer tissue,tumor-adjacent tissue,human gastric cancer cell line SGC7901 and gastric epithelial cell line GES-1 cells were tested by We stern blot.Vav3-siRNA was transfected into the SGC7901 cells. The proliferation of SGC7901 cells in vitro was measured by MTT assay.Cell cycle of SGC7901 cells was determined by flow cytometry.The expressions of proliferation-related genes PCNA,p16,cyclin D1,Rb were determined by qPCR and Western blot assay.Orthotopic transplantation nude mouse models of gastric cancer were prepared,and the tumor growth and expressions of PCNA,P16,cyclin D1,and Rb proteins were examined. Results The relative expressions of Vav3 in the gastric cancer and peritumoral tissue were 0.910±0.242 and 0.243±0.045,respectively;the relative expressions of Vav3 in SGC7901 and GSE-1 cells were 0.925±0.127 and 0.277±0.038,respectively(both P<0.05).The expression of Vav3 protein in SGC7901 cells was effectivelyinhibitedbyVav3-siRNA.Proliferationof SGC7901 cells was inhibited by(83.43±10.17)%after 80 nmol/L Vav3-siRNA transfection(P<0.05).The ratio of SGC7901 cells in G0/G1phase was increased,and in S phase decreased after Vav3-siRNA transfection (both P<0.05).The expressions of PCNA and cyclin D1 were decreased in cells after Vav3-siRNA transfection,and expressions of p16 and Rb were increased after Vav3-siRNA transfection(P<0.05 for all).The tumor growth in the Vav3-siRNA group was much slower than that in the other 2 control groups of nude mouse models. Compared with the two control groups,expressions of PCNA and cyclin D1 were significantly lower in the Vav3-siRNA group,while expressions of p16 and Rb were increased(P<0.05 for all).Conclusion Vav3 can promote the proliferation of gastric cancer cells by regulating proliferation-related genes.

Objective To determine the relationship between serum amino acid composition and pathogenesis of ulcerative colitis(UC),so as to explore some new biomarkers for early diagnosis and treatment.Methods This study was conducted in the 309thHospital of Chinese People's Liberation Army between October 2012 and November 2013.Forty-four UC patients and 52 healthy controls were enrolled and the serum samples were collected.U-sing high performance liquid chromatography-mass spectrometry technology(HPLC-MS/MS)stable isotope internal standard method,we detected serum amino acid metabolic profiling in the two groups.Results Compared with the control group,the serum levels of glutamic acid,glutamine,methionine,tryptophan and histidine in UC group were significantly lower[(59.20±21.93)μmol/ L vs(88.14±34.85)μmol/L;(2 200.51±648.03)μmol/L vs(2 664.91±1 034.74)μmol/L;(268.69± 211.64)μmol/L vs(431.48±298.00)μmol/L;(68.83±32.33)μmol/L vs(89.96±29.29)μmol/ L;(101.88±32.01)μmol/L vs(115.10±17.84)μmol/L respectively,all P<0.05].However,the serum levels of asparagine and isoleucine in UC group were significantly higher than the control group[(195.14± 122.14)μmol/L vs(140.49±34.91)μmol/L;(94.61±29.76)μmol/L vs(80.99±19.73)μmol/L respectively;both P<0.05.]Conclusion The amino acid metabolic profiling in UC patients is different from that in healthy controls,which suggests that there might be a certain relationship between amino acid composition and pathogenesis of UC.Some amino acids would be tested as potential biomarkers in patients with UC.
