鸦胆子油自微乳化颗粒对大鼠长期毒性实验*

鸦胆子油自微乳化颗粒对大鼠长期毒性实验*

目的考察鸦胆子油自微乳化颗粒对大鼠的长期毒性。方法将120只斯泼累格·多雷(SD)大鼠随机分为空白对照组、鸦胆子油自微乳化颗粒大剂量组、中剂量组、小剂量组,每组30只。鸦胆子油自微乳化颗粒大、中、小剂量组分别按照144,72,36倍人体服用剂量给药,并同时进行空白对照。于给药前、给药3,6个月及停药后4周进行大鼠体质量、血液学、血液生化学、脏器指数、病理组织学检查。结果一般观察指标及体质量均未见显著变化:血液学指标:3个月时与空白对照组比较,大剂量组淋巴细胞比率(LY)降低、单核细胞比率(MO)升高,中剂量组MO、嗜碱性细胞比率(BA)均显著升高(P<0.05),给药6个月后与空白对照组比较,大剂量组红细胞容积(HCT)比率降低、中剂量组中性粒细胞计数(NE)升高(P<0.01);停药4周后,以上项目均恢复,与空白对照组比较,差异无统计学意义(P>0.05)。血液生化学指标:3个月时与空白对照组比较,大剂量组总蛋白(TP)、球蛋白(G)、丙氨酸氨基转移酶(ALT),中剂量组TP均显著升高(P<0.05),大剂量组天冬氨酸氨基转移酶(AST)极显著升高(P<0.01);给药6个月后大剂量组AST升高、三酰甘油(TG)降低(P<0.05),中剂量组肌酸激酶(CK)降低(P<0.05)、TG极显著降低(P<0.01),小剂量组葡萄糖(Glu)极显著降低(P<0.01);停药4周后,以上指标均可恢复。尸解及脏器指数:停药后大鼠出现肝脏、睾丸迟发型肿大,整个实验过程未出现动物死亡;病理组织检查未见显著变化。结论鸦胆子油自微乳化颗粒长期服用毒性较低,可进一步进行深入研究。

鸦胆子油自微乳化颗粒;毒性,长期

鸦胆子油是从苦木科植物鸦胆子[Bruceajavanica(L.)Merr.]干燥成熟果实中提取的脂肪油,现代药理学研究证明其具有明确抗肿瘤作用[1]。临床常用鸦胆子油乳注射液、鸦胆子油口服乳剂等剂型治疗肺癌、胃癌等多种恶性肿瘤[2-3]。但上述剂型存在稳定性差、生物利用度低、依从性差等问题,且近年出现不少有关含鸦胆子油的注射液剂不良反应的报道,有研究表明大剂量、长期应用鸦胆子油乳注射剂会使大鼠出现肝脏和肾脏功能损伤及急性肺水肿、溶血性贫血等[4]。本课题组前期进行了鸦胆子油固体自微乳制剂开发研究,研制出鸦胆子油自微乳化颗粒并进行了相关药学研究[5-9]。笔者在本实验中对其进行了长期毒性考察,为其安全性评价及临床用药开发提供基础。

1 材料与方法

1.1 动物 斯波累格·多雷(Sprague Dawley,SD)大鼠,体质量100~140 g,6~8周龄,由第四军医大学实验动物中心提供,合格证号:0022211,生产许可证号:SCXK(军)2007-007。实验开始时体质量差异不超过或低于该实验动物平均体质量的20%。动物净化室温度保持在20~25 ℃,相对湿度保持在45%~70%,保持门窗密闭,使用空气净化系统,送新风全口开启,送风机隔日换机一次,饲料和垫料由钴60照射处理。实验前大鼠正常饲养1周,观察记录大鼠行为活动,饮食、体质量、精神状况,选择健康,正常,若雌性应以无孕大鼠作为受试动物。

1.2 试药 鸦胆子油自微乳颗粒剂为实验室自制,批号:20120218,每袋30 g,每袋含鸦胆子油0.5 g。

1.3 仪器 BS-300型全自动生化分析仪(深圳迈瑞生物医疗电子股份有限公司);Sysmex XS-800i型血液分析仪(深圳迈瑞生物医疗电子股份有限公司);XD-685型电解质分析仪(上海讯达医疗仪器有限公司);BS210S型电子天平(北京赛多利斯天平厂,量:0.1 mg)。

1.4 动物分组与给药 选取120只健康SD大鼠,随机平均分为4组,雌雄各半。分别为鸦胆子油自微乳化颗粒大、中、小剂量组及空白对照组。给药组分别按144,72,36倍人体服用剂量灌胃给予鸦胆子油自微乳颗粒剂(以鸦胆子油计剂量分别为鸦胆子油3.6,1.8,0.9 g·kg-1),空白对照组灌胃给予赋形剂。

1.5 一般观察 给药期间每周称体质量、食量各一次,观察有无死亡或濒死大鼠及大鼠毛色、行为活动、腺体分泌、呼吸、粪便性状等。

1.6 血液学指标检测 在给药3,6个月及停药4周后,各组大鼠分别取10只,采用10%乌拉坦1 mL·(100 g)-1腹腔注射麻醉,分离颈总静脉,取血,采用Sysmex XS-800i血液分析全自动血液细胞分析仪测定白细胞总数(total white blood cells,WBC)、中性粒细胞计数(neut,NE)、淋巴细胞比率(lymphocyte rate,LY)、单核细胞比率(monor rate,MO)、嗜酸性细胞比率(eosinophils rate,EO)、嗜碱性细胞比率(basic cells rate,BA)、红细胞计数(red blood cell count,RBC)、血红蛋白(hemoglobin,HGB)、红细胞压积(hematocrit,HCT)、平均红细胞体积(mean corpuscular volume,MCV)、平均血红蛋白含量(mean cell hemoglobin,MCH)、平均血红蛋白浓度(mean corpuscular hemoglobin concentration,MCHC)、血小板计数(platelet count,PLT)、网织红细胞(reticulocyte,RET)、凝血时间(coagulation time,Coag-T),共计15项。

1.7 血液生化学指标检测 采用上述同样方法取血,采用BS-300全自动生化分析仪检测:总胆红素(total bilirubin,T-BiL)、总蛋白(total protein,TP)、清蛋白(albumin,ALB)、球蛋白(albumin,G)、ALB /G、丙氨酸氨基转移酶(glutamic-pyruvic transaminase,ALT)、天冬氨酸氨基转移酶(glutamic-oxalacetic transaminase,AST)、 AST/ALT、碱性磷酸酶(alkaline phosphatase,ALP)、葡萄糖(glucose,Glu)、总胆固醇(total cholesterol,TC)、三酰甘油(glycerin trilaurate,TG)、尿素氮(blood urea nitrogen,BUN)、肌酐(creatinine,Crea)、肌酸激酶(creatine kinase,CK),共计15项。

1.8 系统尸解及脏器指数 对大鼠进行系统尸解,全面细致地观察脏器,并详细记录病变脏器的外观特征。计算心、肝、脾、肺、肾、肾上腺、胸腺、睾丸、附睾、子宫、卵巢、脑12个脏器的脏器系数,并进行统计分析。

1.9 病理组织学检查 光学显微镜检查,内容包括:心、主动脉、肺、气管、食管、胃、肝、胰腺、小肠、大肠、颌下腺、肾、膀胱、子宫、卵巢、阴道、睾丸、附睾、前列腺、肾上腺、甲状腺、垂体、脑(大脑、脑干、小脑)、脊髓、坐骨神经、脾、胸腺、肠系膜淋巴结、骨髓、皮肤共32个脏器和组织。

2 结果

2.1 一般观察结果 给药期间无死亡或濒死动物,大鼠毛色光洁,行为活动、腺体分泌、呼吸、粪便性状均正常。3个剂量组大鼠体质量及摄食情况均正常,与空白对照组比较,差异无统计学意义(P>0.05)。见图1。

2.2 血液学指标检测结果 3个月时与空白对照组比较,大剂量组LY降低、MO升高,中剂量组MO、BA均显著升高(P<0.05),6个月时与空白对照组比较,大剂量组HCT比率降低、中剂量组NE升高(P<0.01),但在停药4周后,以上项目均可恢复,与空白对照组比较,差异无统计学意义(P>0.05)。结果表明鸦胆子油自微乳化颗粒长期用药可能会引起血液学指标的可逆性变化。见表1。

2.3 血液生化学指标检测结果 3个月时与空白对照组比较,大剂量组TP、G、ALT,中剂量组TP均显著升高(P<0.05),大剂量组AST极显著升高(P<0.01)。6个月时大剂量组AST升高、TG降低(P<0.05),中剂量

组CK降低(P<0.05)、TG显著降低(P<0.01),小剂量组GLU显著降低(P<0.01)。但是在停药4周后,以上指标均可恢复,其他指标在实验期间,与空白对照组比较,差异无统计学意义。表明鸦胆子油自微乳化颗粒长期用药可能会引起可恢复性肝脏损伤。见表2。

2.4 系统尸解及脏器指数结果 鸦胆子油自微乳化颗粒长期服用期间对大鼠脏器指数无明显影响,停药后大剂量组肝指数、睾丸指数明显升高。提示鸦胆子油自微乳化颗粒大剂量可能引起迟发性质肝脏、睾丸肿大。见表3。

2.5 病理组织学检查结果 实验期间无动物异常死亡,所有动物均存活至实验结束时间,按计划解剖腹主动脉放血处死。

肉眼观察:各组动物皮肤完整,未见脱毛、红肿等皮肤病变;耳、口、鼻、肛门等天然孔道无分泌物和外伤;眼睛呈粉红色、无发绀、黄染及红点;皮下组织无出血和肿物,肌肉颜色粉红,有光泽并富于弹性;胸、腹、颅腔无液体或气体潴留、无异味,各脏器位置和形态正常,无粘连。病理学检查未发现药物所致的特异性毒性反应。

光学显微镜检查:给药3和6个月结束时,在大剂量组分别发现大鼠肾小管间质局灶性充血和肝静脉周围局灶性变形、坏死各1例。其他大鼠在同时间段未发现异常病变。另外给药6个月以及恢复期过程中,空白对照组及大剂量组肺脏、肝脏及肾脏均出现1或2例轻度炎症,应为成年大鼠自发性症状,无特异性毒性反应[10]。综上,鸦胆子油自微乳颗粒剂给药6个月病理学检查,未发现明显靶器官毒性。

3 讨论

有报道,鸦胆子油乳注射液大剂量(1 g·kg-1)长期给药会引起大鼠稀便、皮毛无光泽、精神萎靡,引起大鼠肝脏、肾脏急性损伤和肺水肿,死亡率7.5%[4]。

笔者在本实验中,将大鼠分别给予大、中、小剂量鸦胆子油自微乳化颗粒(分别为人体服用剂量的144,72,36倍)6个月,一般观察指标及体质量均未见显著变化;大、中剂量组血液学指标(LY、MO、BA、NE、RBC)出现可逆性变化;大剂量出现肝脏、肾脏损伤(停药后可恢复)及肝脏、睾丸迟发型肿大,整个实验过程未出现动物死亡。以上实验结果证明鸦胆子油自微乳化颗粒毒性低,可进一步进行深入研究。

Long-term Toxicity of Self-microemulsifying Granules of Brucea Javanica Oil on Rat

Objective To test long-term toxicity of self-microemulsifying granules ofBruceajavanicaoil on SD rats . Methods The SD rats were divided into blank control group, high-, medium- and low-dose (144, 72 and 36 folds of dosage for human, respectively) of self-microemulsifying granules ofBruceajavanicaoil groups randomly (n=30 each group).The weight, appetite, haematological indexes, blood biochemical indexes, visceral index and histopathologic changes of organs of rats were measured at time point after 3, 6 months treatment and 4 weeks after drug withdrawal, respectively. Results Compared with blank control group, the drug groups showed no significant differences in the appearance and weight of rats.With haematological indexes, the level of lymphocyte rate (LY) in high dose group was significantly lower while the levels of monor (MO) in high dose group was significantly higher, MO and basic cells rate(BA)in medium dose group were significantly higher than those in blank control group (P<0.05) after 3-month long treatment.Hematocrit (HCT) in high dose group was significantly lower, neut (NE) in medium dose group was very significantly higher than those in blank control group(P<0.01) after 6-month long treatment.Those changed indexes recovered to be blank on the 4th week after withdrawal.No significant difference was obtained compared with blank control group(P>0.05).In blood biochemical indexes, the levels of total protein (TP), albumin (G), glutamic-pyruvic transaminase (ALT) in high dose group and TP in medium dose group were significantly higher than those in blank control group (P<0.05).Glutamic-oxalacetic transaminase (AST) in high dose group was very significantly higher than that in blank control group (P<0.01) after 3 months.After 6 months treatment , compared with blank control group, the level of glutamic-oxalacetic transaminase (AST) in high dose group was higher while trilaurate glycerin (TG) was lower(P<0.05).The level of creatine kinase (CK) was lower(P<0.05) and the level of TG was very significantly lower (P<0.01).The level of glucose (Glu) was very significantly lower (P<0.01).Those changed indexes recovered to be blank on the 4th week after withdrawal.In haematological indexes,delayed enlargements of liver and testis occurred after withdrawal.No rat died in the process of experiment.No obvious histopathologic changes of organs of rats were observed . Conclusion Self-microemulsifying granules ofBruceajavanicaoil have no apparent toxicity.It is worthy of further research.
