K3机用镍钛锉与K锉的临床应用比较

【摘要】 目的:比较K3机用镍钛锉与手用K型锉的临床应用。方法:选择285颗患牙(失查数5颗),共526个根管,随机数字表法分为K3组和K锉组,分别用K3机用镍钛锉和K锉进行根管预备,比较两组患者根管预备时间、根管预备术后疼痛反应和根管充填质量。结果:K3组所用时间明显少于K锉组;K3组根管预备后疼痛发生率为(5.51%),较K锉组的根管预备后疼痛发生率(10.24%)明显降低(P<0.01);K3组根管恰填率为(94.18%),较K锉组(86.22%)明显提高(P<0.01)。结论:K3机用镍钛锉预备根管具有效率高、成形能力强、根充效果好、明显减少术后疼痛等优点,是一种理想的根管预备器械,值得临床上广泛应用。

【Abstract】 Objective: To make a comparison between K3 rotary nickel-titanium files and hand operated K files. Method: 285 bad teeth(5 missing), totally with 526 incisive canals were divided into K3 group and K file group randomly, for which root canal preparations were made respectively with rotary nickel-titanium files and K files, to compare the time of root canal preparation, pain reaction after the preparation and filling quality of root canal within each group. Result: The result proved that the root canal preparation time using rotary nickel-titanium files of the K3 group was much less than that of the K file group. The rate of pain reaction after K3 preparation was 5.51%, which was less than that of K file group (10.24%) (P<0.05), and the well-filled rate of K3 group was 94.18%, which was more than that of the K file group (86.22%) (P<0.05). Conclusion: Rotary nickel-titanium files have high efficiency, strong shaping ability, good root filling effect, which can significantly reduce postoperative pain, ideal root canal preparation equipment, worthy of clinical application.

在治疗牙髓炎和根尖周炎方面,根管治疗术(root canal trerapy, RCT)是最有效、最常用的方法,其中根管预备的成形决定着根管治疗的成败,根管预备的质量是根管治疗术能否成功的关键。传统的手用不锈钢K锉预备根管时,由于该合金本身固有的硬度和刚度,在预备弯曲狭窄根管时,常常造成根管变形,根尖偏移,而且费时费力。新型镍钛根管预备器械K3因其具有超弹性和柔韧性,切削效率高,成形效果好,克服了不锈钢器械的缺点,得到迅速推广及应用。
1 资料与方法
1.1 一般资料 选择2011年5月-2012年3月在锦州市口腔医院就诊行根管治疗的270例患者,年龄25~65岁,其中男156名,女114名,患牙总数285颗(失查数5颗),共526个根管,均为前磨牙和磨牙,由同一名医生完成根管预备及充填。随机分为K3组患牙143颗(失查数3颗),共272个根管;K锉组患牙142颗(失查数2颗),共254个根管。两组患者性别、年龄、牙位构成的差异无统计学意义(P>0.05),具有可比性。纳入标准:(1)临床上确诊为牙髓疾病及根尖周疾病的恒牙;(2)无治疗史;(3)无根折及根管内吸收的;……
