基层医院门诊处方不合理用药分析

目的：对本院1200张处方进行点评，分析其中典型不合理处方，了解门诊用药情况，促进临床合理用药。方法在本院2012年3月至2013年2月所有处方中每月随机抽取100张，共计1200张，以《医院处方点评管理规范(试行)》为依据，对每张处方中的不合格问题进行归类、总结，并针对部分不合理处方进行分析。结果不合格处方205张，占17.08%，主要表现为用法用量不合理、重复用药、存在配伍禁忌、无适应症用药等。结论本院门诊用药基本合理，但仍存在一定问题，应当继续加强宣传与管理，完善相应的规章制度，增加与临床医师交流，及时干预不合理用药。

Abstract: [Objective] To analyze typical unreasonable prescriptions, learn about outpatient medications, and promote rational clinical drug usage through reviewing 1200 prescriptions. [Method]1200 prescriptions were randomly selected (100 prescriptions per month) from March 2012 to February 2013. Unqualified prescriptions were analyzedand summarized according to “Comments on the management of hospital prescription specification (Trial)”. [Result]205 were unqualified prescriptions (17.08%). Unreasonable usage and dosage, excessive dosage, incompatibility, un-targeted usage were the main causes for unqualified prescriptions. [Conclusion]The outpatient prescription quality of our hospital is basically reasonable, but there are still some issues that need to be addressed by stepping up publicity and management, improving corresponding rules and regulations, increasing communication with clinicians, and timely intervention of irrational drug use.

处方点评及不合理处方分析是了解临床用药情况和促进临床合理用药的重要措施之一。卫生部于2007年5月1日实施了《处方管理办法》[1]，明确规定医疗机构应当建立处方点评制度，药师应当对处方适宜性进行审核、登记并通报不合理用药后给予干预。为规范处方管理，提高处方质量，促进合理用药，笔者按照卫生部印发《医院处方点评管理规范(试行)》[2]等要求，对我院2012年3月至2013年2月门诊处方进行随机抽样，开展处方点评，评价门诊用药合理性，指出其中存在的问题，为临床合理用药提供建议。
