[摘要]目的：探討Q开关1 064nm Nd：YAG激光联合珍珠美白散治疗黄褐斑的临床疗效。方法：选取2016年4月-2019年4月在笔者医院接受治疗的黄褐斑患者110例作为研究对象，采用随机数字表法将其分为观察组（n=56）和对照组（n=54）。观察组采用Q开关1 064nm Nd：YAG激光联合珍珠美白散治疗，对照组采用Q开关1 064nm Nd：YAG激光治疗。对比两组临床疗效、皮损总积分、积分下降指数、不良反应率及复发率。结果：治疗后，观察组临床总有效率明显高于对照组（92.86 vs 66.67）%（P<0.05）;观察组皮损总积分明显低于对照组（1.27±0.26 vs 2.87±0.38）分，且积分下降指数明显高于对照组（0.66±0.07 vs 0.52±0.06）（P<0.05）;观察组不良反应率明显低于对照组（3.57 vs 53.70）%（P<0.05）;观察组复发率明显低于对照组（1.79 vs 18.52）%（P<0.05）。结论：Q开关1 064nm Nd：YAG激光联合珍珠美白散治疗黄褐斑临床疗效理想，有效改善皮损状况，且对降低复发率及不良反应率效果显著，值得在临床上推广及应用。

Abstract： Objective  To Study on the clinical efficacy of Q switch 1 064nm Nd：YAG laser combined with pearl whitening powder in the treatment of chloasma. Methods  110 cases of chloasma patients who received treatment in our hospital from April 2016 to April 2019 were selected as the study subjects， and were divided into observation group （n=56） and control group （n=54） by random number table method. The observation group was treated with Q switch 1 064nm Nd：YAG laser combined with pearl whitening powder， while the control group was treated with q-switch 1 064nm Nd：YAG laser. The clinical efficacy， total score of skin lesions， index of decline in score， adverse reaction rate and recurrence rate were compared between the two groups. Results  After treatment， the total clinical effective rate of the observation group was significantly higher than that of the control group （92.86vs66.67）% （P<0.05）.The total score of skin lesions in the observation group was significantly lower than that in the control group （1.27±0.26 vs 2.87±0.38）， and the index of decline was significantly higher than that in the control group（0.66±0.07vs 0.52±0.06）（P<0.05）. The adverse reaction rate in the observation group was significantly lower than that in the control group （3.57 vs 53.70）% （P<0.05）.The recurrence rate in the observation group was significantly lower than that in the control group （1.79 vs 18.52）% （P<0.05）. Conclusion  Q switch 1 064nm Nd：YAG laser combined with pearl whitening powder has an ideal clinical effect in the treatment of chloasma， which can effectively improve the condition of skin lesions and significantly reduce the recurrence rate and adverse reaction rate. Therefore， it is worth promoting and applying in clinical practice.

美白散全方有活血化瘀，行气健脾，利润美白之功[17]。珍珠美白散是笔者医院皮肤科根据多年的临床经验研发出的一个中药外用方。由珍珠粉、白僵蚕、白芷、白茯苓、白芨、玫瑰花及白蒺藜等药物组成。其中珍珠粉可令皮肤柔滑白嫩、收敛生肌;白芷可散风除湿，通窍止痛;白僵蚕可祛风止痒，散结解毒;白蒺藜可平肝明目、疏肝郁结;白茯苓可利水渗湿、健脾补中、宁心安神;白芨可止血消肿、善治面疮;玫瑰花气味芬芳，调和药物，具有较强的成膜性[18-19]。以上药方均有疏肝健脾、行气活血、祛斑美白之功，可奏润肤美白，祛黑消斑之效。因此，笔者医院采用Q开关1 064nm Nd：YAG激光联合珍珠美白散治疗黄褐斑。珍珠美白散用以提高黄褐斑患者的临床疗效及减少复发率的发生，同时使患者皮肤更具光泽度，增添患者自信[20]。

本研究结果显示，Q开关1 064nm Nd：YAG激光联合珍珠美白散治疗黄褐斑患者可有效提高临床疗效，联合治疗临床总有效率92.86%，明显高于单一采用激光治疗后临床总有效率66.6%;从图1可以看出，联合治疗后黄褐斑明显消退，色泽光亮，还有美白效果，单一激光治疗后的效果不明显，且色素暗沉。同时联合治疗后的皮损总积分显著低于单一采用激光治疗后的皮损总积分，积分下降指数显著高于单一采用激光治疗;联合治疗后的不良反应率3.57%明显低于单一治疗的不良反应率53.70%;半年后，随访发现单一治疗的复发率18.52%，明显高于联合治疗后的复发率1.79%，两种治疗方法差异显著，说明联合治疗的效果更佳，是一种安全、有效的治疗方法。

综上所述，Q开关1 064nm Nd：YAG激光联合珍珠美白散治疗黄褐斑患者，对提高临床疗效、降低复发率、预防激光治疗后炎症性色素沉着效果显著，且不良反应少，美白效果理想，使用方便，值得在临床上推广应用。
